Pope Leo XIV has been diagnosed with lung adenocarcinoma. While we had hoped for a different diagnosis, the fact that he was able to undergo a lobectomy is encouraging. Surgery is generally pursued when lung cancer appears localized and potentially curable. Final pathology will help determine prognosis and whether additional treatment is needed. Wishing the Holy Father a smooth and speedy recovery. lombardi.georgetown.edu/lomb…

Human! Know thyself!
Pick any medical specialty other than your own. Who’s the most famous physician in it? You probably can’t name one. Almost none of us will be known outside our field, and even those who achieve real professional fame are mostly forgotten within a few decades. Fame is a poor reason to devote your life to medicine. The work itself matters more: caring for patients, discovering new knowledge, and teaching the next generation.

Sharing our recent manuscript highlighting DLL3 IHC predictive of tarlatamab response and long-term outcome, check this out! Key findings: 1. DLL3 IHC expression in patient tissue ranging widely from 0% to 100% (median of 70% and 75%, in two datasets) 2. DLL3 expression above 75% experienced a significantly longer median time on tarlatamab (ToT)treatment (10.1 months vs. 2.9 months) compared to those with ≤75% expression. 3. Patients achieving a PR to tarlatamab had a higher median DLL3 expression level (80%) than those with PD (40%) biorxiv.org/content/10.64898… @LaurenByersMD @LauraAlderMD @lungoncdoc @drshieldsmd @kyleconcannonMD @SclcSMASHERS k

A positive ctDNA result does not necessarily mean MRD. In NSCLC surveillance, 59% of patients with a positive methylation-based ctDNA result subsequently became negative, mostly without radiologic recurrence. Low-level signals around the assay’s detection limit were particularly unstable. In contrast, all three patients with clearly higher ctDNA fractions had confirmed recurrence. As ctDNA increasingly enters perioperative trials and potentially treatment decisions, perhaps the clinically relevant biomarker is not simply ctDNA+/−, but its level, persistence, and trajectory. A single low-level positive result may be a poor reason to label a patient as having molecular residual disease and escalate treatment. This is a small retrospective study, but an important warning against reducing MRD to a binary variable. I raised a similar concern when discussing SERENA-6. Unfortunately, it did not attract the editor’s interest at the time and was therefore not published in a “scientific journal.” Given the sometimes difficult-to-understand discretionary nature of editorial decisions, I have increasingly chosen to share some of my scientific views independently, as I did in the post below. jtocrr.org/article/S2666-364…
Thoughts on SERENA-6 is a personal commentary that has not been published in conventional academic journals. From now on, I will share my personal writings in this format as a protest against a publishing system shaped by arbitrary editorial decisions, editors’ personal biases and conflicts of interest, favoritism within professional networks, excessive article processing charges, and the coercive “publish or perish” culture.

Time to start understanding mechanisms of resistance to zongertinib, which is rapidly becoming the standard of care (efficiency/toxicity ratio and BM efficacy) for our #HER2-mut NSCLC pts. Sequential tissue biopsies are key, but remain highly challenging.#WCLC2026

Excited to share updates on the latest in NSCLC!
Looking forward to sharing perioperative strategies in early-stage #NSCLC at an upcoming State of the Science Summit. We'll walk through the neoadjuvant vs perioperative data, the borderline-resectable MDT question, and what the new subcutaneous IO approvals mean in practice. Always more to learn in this fast-moving field. #LCSM #LungCancer Join us tomorrow in Scottsdale!
