NTSB Points to Situational Awareness Failure in Dallas Medevac Rooftop Helipad Crash
The National Transportation Safety Board has identified a lack of situational awareness as a causal factor in the May 13 crash of an Air Evac Lifeteam Bell 407 in Dallas.
The accident occurred during a night takeoff from a rooftop helipad. All three crew members on board were uninjured, but the helicopter sustained substantial damage to its tail rotor.
