My mother nearly died from a bleeding peptic ulcer caused by H. pylori. For me, stopping GI bleeding has always been deeply personal.
That experience gives added meaning to the privilege of speaking at the American College of Gastroenterology’s Annual Postgraduate Course and moderating “GI Bleeding Unplugged: Real-World Tools and Techniques.”
My talk focused on advances in endoscopic treatment of upper GI bleeding—from hemostatic topical agents to over-the-scope clips—and how to use these tools when the stakes are highest.
Behind every bleeding ulcer is a person, a family, and a future at risk.
Why the hat? Nashville inspired me. Channel your inner cowboy—be brave and try something new if that’s what it takes to save a life.

▶️Risk of CRC low in both groups
▶️No difference in cancer risk in 3 vs 5 year surveillance

The problem isn’t that physicians can’t manage time.
The problem is that we’ve designed careers that assume unlimited access to our evenings.
A few changes to workflow, boundaries, and schedule design can give hours back every week.
Not all work is inevitable.

Replying to @NeenaSAbrahamMD
Physicians have boundary issues. We are constantly encouraged to have maximum availability.
It’s not healthy.

Most physicians think they need a different job.
Often, they need a different structure.
The problem isn’t always medicine. It’s the accumulation of after-hours charting, endless committees, inbox creep, and responsibilities nobody taught you how to contain.
You don’t need more resilience.
You need boundaries.

Most physicians don’t have a time-management problem.
They have a career structure problem.
The biggest surprise in coaching isn’t how much work follows doctors home.
It’s how much of it never should have.

👉🏽 The best leaders disappoint people regularly.
Not because they’re difficult.
👉🏽 Because they understand every yes creates a new obligation.
👉🏽 Leadership requires protecting priorities.

The average human life is ~4,000 weeks.
Yet so much of medicine is spent trying to get “caught up” - as if one day the inbox, task list, and backlog will finally disappear.
Today’s reminder in @HorstIBDDoc talk: the backlog is permanent.
Leadership isn’t about doing more. It’s about choosing what deserves your finite time.
Every “no” creates space for a deeper “yes” - to your patients, your team, your family, and the work that matters most.
Success isn’t getting everything done. It’s being intentional about what you choose to do.

This piece by @HorstIBDDoc and @ibdgijami in @AGA_GIHN says something we don’t say out loud enough in GI.
The quietly qualified.
Highly capable. Deeply invested. Largely unseen.
“Self-promotion is not about ego. It is about stewardship of one’s skills, clarity of purpose, and ensuring that excellence is recognized.”
That line is exactly right. And the data backs it up: despite growing numbers of women entering academic medicine, advancement from associate to full professor has barely moved in 35 years.
Salary gaps persist.
The pipeline is full.
The top is not.
This is not a talent problem.
It is a visibility problem.
This weekend I’ll be speaking on exactly this topic at the @AmCollegeGastro Early Career Leadership Program
Brag Because You Earned It: Strategic Visibility for Early-Career Gastroenterologists.
Read it before the weekend. Then let’s talk.

Physicians are taught that drawing attention to one’s accomplishments is inappropriate, but self-promotion helps leaders consider you for future opportunities. @HorstIBDDoc & @ibdgijami share how the "quiet qualified" can learn self-promotion as a skill. ow.ly/O1lc50Z4UQq

Among patients with previously treated metastatic pancreatic ductal adenocarcinoma, the RAS(ON) inhibitor daraxonrasib led to significantly longer overall survival and progression-free survival than chemotherapy. Full phase 3 RASolute 302 trial results: nej.md/4nWaxvM

ALT Graph from a study published in the New England Journal of Medicine on daraxonrasib versus chemotherapy for metastatic pancreatic cancer. The x-axis represents months, and the y-axis shows the percentage of patients alive. Two lines indicate survival rates: daraxonrasib (blue) and chemotherapy (gray). The bottom section lists patients at risk over time. Title and journal logo are included.

Physicians make life-or-death decisions all day, then stand in front of the fridge unable to decide what’s for dinner.
👉🏽 That’s not laziness.
👉🏽 That’s decision fatigue.
The answer isn’t making better decisions.
It’s making fewer of them.
