The knowledge gap between colleagues in different medical specialties is often strikingly humbling, yet this humility does not arise from inferiority or lack of competence. Instead, it emerges from the realization that what appears to be a gap is in fact the product of intentional divergence in focus.
Medical specialization trains clinicians not only to know different things, but to see differently, to prioritize distinct signals, apply unique interpretive frameworks and operate on different scales of time, anatomy and meaning.
Each specialty develops depth by narrowing its field of attention and it is this narrowing that creates the illusion of absence when viewed from the outside.
The humbling moment occurs when one confronts the limits of one’s own trained lens and encounters the extraordinary clarity of another’s. In that moment, the clinician recognizes that medicine is not a hierarchy of knowledge, but a system of distributed cognition, where no single mind is meant to hold the whole picture. What feels like a personal inadequacy is actually a structural necessity of modern medicine.
The perceived knowledge gap is therefore not a deficit, but evidence of a sophisticated design in which collective understanding emerges only through collaboration.

I strongly support rich people marrying rich people. It’s not about elitism. It’s about making life and money work better together. Money isn’t just income. It’s habits, risks, assets and long-term thinking. When two people with similar financial habits marry, their lives run smoother instead of clashing.
Wealth sticks around when both partners understand saving, investing and planning ahead. Fights over money drop. Stress drops. Life gets simpler.
Class matters. Wealth shapes how people see time, risk, security and opportunity. When financial mindsets don’t match, tension and resentment appear. Alignment avoids that.
Marriage also merges networks. Connections, influence and opportunities grow when both partners come from similar economic circles. That’s practical not romantic but it is not to their can't be romance.
Lifestyle expectations must match. Some think in decades, others in paychecks. If your timelines clash, stability suffers even if the love is real.
Families with money think generations ahead. Marrying within the same class makes estate planning, obligations, and peace of mind easier.
Love can cross class lines. But the mentality of the person from the lower class should be aligned enough as not to create certain conflicts. When building and preserving wealth, alignment matters most.
Elevate your self or marry at your level and if you are opportuned to marry up, it is important you build the capacity and mentality before hand to operate at that level and circle.

Specific Antibody Deficiency (SAD) (also known as Selective Antibody Deficiency or Impaired Response to Polysaccharide Antigens)
Key Clinical Features
Definition: Specific Antibody Deficiency (SAD) is a primary immunodeficiency characterized by normal absolute levels of total immunoglobulins (\text{IgG}, \text{IgA}, \text{IgM}) and normal \text{IgG} subclass levels, but a specific inability to produce functional protective antibodies against polysaccharide antigens (such as encapsulated bacteria like Streptococcus pneumoniae, Haemophilus influenzae type b, and Neisseria meningitidis).
Clinical Presentation: Recurrent upper and lower sinopulmonary tract infections (e.g., recurrent otitis media, chronic sinusitis, bronchitis, and pneumonia) in children or adults despite normal quantitative baseline immunoglobulin levels.
Diagnostic Standard:
Baseline anti-pneumococcal antibody titers are measured.
The patient is immunized with the 23-valent pneumococcal polysaccharide vaccine (PPSV23 / Pneumovax).
Post-vaccination titers are measured 4\text{--}6 weeks later. A failure to mount a 4-fold or protective increase in titers to a majority of serotypes confirms the diagnosis.
Management Strategy
Prophylactic Antibiotics: Daily low-dose prophylactic antibiotic therapy (e.g., Amoxicillin or Trimethoprim-sulfamethoxazole) is often the initial management strategy.
Conjugate Vaccination: Administration of pneumococcal conjugate vaccines (PCV13 / PCV20), which recruit T-cell-dependent immune responses and may elicit protective antibody responses where pure polysaccharide vaccines fail.
Immunoglobulin Replacement Therapy (IgRT): Intravenous (IVIG) or subcutaneous (SCIG) immunoglobulin replacement is indicated for severe cases, refractory infections, or progression to bronchiectasis.

It is official 🎉. I am a PhD Candidate! Still taking this one in as it feels especially special.
I started my PhD three weeks postpartum at @queensu, with a newborn in my arms and absolutely no idea what it would take to balance motherhood, life, and doctoral studies. It wasn’t easy, but here we are. That tiny baby is now two years old, surrounded by powerful women he has come to know as his “PhD aunties”.
I successfully defended my PhD thesis proposal, and I am so excited for what comes next. To my family, thank you for carrying me and making room for this dream. To my supervisor, Dr. @KTerna , thank you for giving me the space to grow into my own voice as a scholar. Your mentorship throughout this journey has meant more to me than I can say. To my wonderful committee, Dr. @pilar_camargo and Dr. @dmacdonald902 , and my examiners, Dr. Katherine McKittrick and Dr. Katie Goldie, thank you for challenging my thinking with support and guidance. I am blessed with the most supportive classmates who created time out of their busy schedules to listen to me rehearse and checked in to encourage me.
Now comes the part I have been most excited about: the research itself. I get to listen to and learn from Black women as I explore their experiences and meaning-making of miscarriage in Canada. Being a survivor of miscarriage and stillbirth, I feel so privileged to carry this work forward. What a beautiful milestone! From three weeks postpartum to PhD Candidate 🎉🙏🏿

A 66-year-old man presents with a 1-week history of diarrhoea and vomiting. He complains of severe back pain and generalized weakness and is admitted to the emergency department. Examination reveals reduced power in his lower limbs, ataxia, tachypnoea, and diplopia. A CT scan of the brain is normal. Which one of the following is the next most appropriate investigation?
A. Urine dipstick
B. Magnetic resonance imaging (MRI) of the brain
C. Echocardiogram
