♀️Most women who get waved off in perimenopause are not dramatic. A lot of clinicians were barely taught this. In one survey, only about 7% of medical residents felt prepared to manage menopause. About 20% got no lectures on it at all. If you were told it was “just stress,” that can be a training gap. Not a personality flaw.

Unfortunate news out of New York as a marathon runner passed away due to rhabdomyolysis complications What is that?⬇️ Rhabdomyolysis (“rhabdo”) is more than extreme muscle soreness. It happens when damaged muscle breaks down and releases substances into the bloodstream that can injure the kidneys and dangerously raise potassium levels. Triggers include intense or unfamiliar exercise, heat, crush injuries, certain medications, alcohol and other drugs, infections, or being unable to move for a long time. 🚩Here’s an important misconception: you don’t need dark urine to have rhabdo. The “classic” combination of muscle pain, weakness and dark urine occurs in fewer than 10% of patients. Don’t wait for all three before getting checked. Severe or unusual muscle pain, marked weakness, swelling, or tea-colored urine—especially after a possible trigger—should prompt urgent medical evaluation. The treatment priorities are stopping the cause and giving early IV fluids, with careful monitoring of kidney function and electrolytes. Commonly used add-ons, such as bicarbonate and mannitol, haven’t shown clear benefit for routine use. And while the muscle enzyme CK helps identify muscle damage, that number alone cannot tell doctors who will develop kidney failure or need dialysis. The whole clinical picture matters.

Great piece in @BrandonLuuMD newsletter If there was any doubt as to why to take the best care of yourself, here’s a good framework

Connelly back on the bench Great news, likely got the wind knocked out of him
Trevor Connelly takes a huge hit and left the ice with trainers Reaching for his side so concern for a rib injury This is a common mechanism for a shoulder/AC joint sprain/separation but he seems to move that left shoulder pretty well after when the trainer gets there Could have just had the wind knocked out of him as well

Fantastic news for Clarke as he probably went though the beginning protocol stages
Brandt Clarke is out on the ice. Good sign that he’s not in a red jersey #GoKingsGo

Cooley is deemed week to week on IR which fits more of the MCL sprain window depending on the grade Good news that the ACL is safe Could be some sort of ankle sprain too but as mentioned it did not seem too bad of a mechanism on the ankle
Devin Cooley of the Flames left the game with a LBI after the collision seen below His left leg extends out and gets caught in an awkward position and he collapses on top of it First glance had me thinking more adductor strain but the way he falls on the leg could mean more knee ligament (MCL/ACL) or meniscus injury It does seem to get too stuck underneath him but it hard to tell Could be looking at a more than a day-to-day situation but hoping that is wrong

The baseball season is over and with a pending lockout an interesting piece about prolonged training interruptions came across my eyes Could be useful for the general population as well⬇️ A review in The American Journal of Sports Medicine found that a complete stop in training costs up to 10% of fitness for each week of inactivity. Aerobic and strength endurance fade faster than speed and maximal strength. Detraining reduces VO2 max, blood volume, stroke volume and insulin sensitivity, and it raises body-fat percentage After four weeks of inactivity, fast-twitch fibres start converting to slow-twitch, and athletes with more muscle to begin with lose it faster, and the loss of flexibility that follows raises the risk of injury The 2020 shutdown is the closest thing we have to a natural experiment In Swiss elite athletes had training times drop which interesting also heightened anxiety, depression, and insomnia Most of these effects returned to baseline once restrictions ended and could be attributed to financial concerns too The moral for you is to NEVER STOP MOVING

Brandt Clarke down and out after this dirty hit Will vertical’s he checked for a concussion and neck injury through the protocol Common mechanism for a potential AC joint sprain/separation but Clarke seems to reach for the jaw/head first so maybe less likely

Spitting up blood or “something like that”????
Christian Watson on what, exactly, happened tonight. He wanted to keep playing immediately, but he was coughing up blood on sideline. Tests checked out at hospital. Just a broken rib. Never stopped saying he’d return, but didn’t know until leaving hospital. Psycho stuff.

Brock Faber has been reportedly cleared to begin skating again in a couple of days after this play left him with a fractured skull about a month ago Him being cleared to skate tells us this was likely an uncomplicated fracture and concussion symptoms may have resolved which is fantastic news There is still a long road for Faber to return to fully playing, likely at the very least a couple more months to begin ramping up any sort of contact Will be interesting to see other potential precautions taken for Faber such as equipment modifications The skull heals well and the reinjury risk lies more in brain injury versus fracturing it again. It is important to note that feeling better does NOT always equal fully recovered, especially in this case

Warren Foegele left the game with assistance last night after knee on knee contact Lauzon’s knee seemed to strike Foegele’s knee on the outside more and causing that valgus stress that is seen very commonly in MCL sprains/tears The knee did not seem to give as much as guys like Marchenko the other day which is promising but cannot rule out any ligament or meniscus injury of the knee Any time there is a big blow like this it is important to rule out a tibial plateau fracture (pictured below) among other fractures, so X-Rays were likely taken A simple contusion is a diagnosis of exclusion so we will wait to see Foegele’s timeline

Noel Acciari left the game after this collision but later returned which is good news It seems the injury came from the knee to the head in what we call an axial loading pattern injury This mechanism in general is concerning for a cervical (neck bone) fracture of spinal cord injury; this definitely did not happen as he returned but is just a general tidbit It can also cause concussions very easily which is likely why Acciari got checked out Wonder if he will deal with some stiffness or neck strain symptoms later on

Another angle of Cooley that leads to more knee speculation versus groin Ankle could even be involved but it didn’t seem to get trapped too

Zach Hyman placed on IR from this hit and reportedly has a “stiff neck” It still could be likely that this is treated as a concussion with a neck injury of some sorts involved I am sure they looked for signs of unstable neck injury and red flag concussion The neck may have undergone a strain of some sorts that has been causing the stiffness mentioned Hyman could just go through concussion protocol at the same time he is healing the neck up

Likely some sort of grade 1 MCL sprain for Marchenko given the mechanism and timeline Given at the time of injury his skate was not stuck in the ice could have prevented an ACL injury with it Grade 1 injuries have returned to play in as little as 10 days but Kirill will probably wait till he is pain free and feels stable
🚨 A group of Leafs fans bumped into Kirill Marchenko at the Calgary airport today, where he told them he was “flying back home today” and will be back in “two weeks.” As of now, no official confirmation has been made by the Leafs (should know more tomorrow) 📸: @Alwaysbardown1 #leafs
