Individuals with Fontan circulation experience marked exercise limitations driven by both central and peripheral deficits—and this decline in peak VO2 accelerates with advancing age post-surgery. This new #CJCPC systematic review/meta-analysis highlights why early functional assessment and targeted exercise interventions are vital. New in #CJC: Mitral leaflet tear after TEER may not be random. Across 4 cases, disruption followed successful first-device implantation and occurred during second-device deployment, suggesting a possible tension-mediated mechanism. Key warning signs: rising transmitral gradient, restricted leaflet motion, and unstable grasping. Pulmonary valve replacement (PVR) after tetralogy of Fallot repair delivers clear short-term wins in PR fraction, RV remodeling, and NYHA class—but long-term meta-regression data remains strikingly sparse. Sustained benefits beyond the early post-op window stay unproven. New in #CJCPC: Concomitant adenovirus in acute Kawasaki disease (KD) doesn't alter diagnostic fidelity or response to IVIG—and coronary outcomes remain excellent. Treat prompt immunomodulatory therapy as standard regardless of viral PCR panel status. Hysterectomy isn’t just a gynecologic event. In population-level data, benign hysterectomy with ovarian conservation was linked to earlier cardiometabolic disease—especially at younger ages. This new #CJC article indicates that it's time to rethink CV risk assessment in women. Read here ⬇️ onlinecjc.ca/article/S0828-2… #OpenAcess #WomensHeartHealth #Prevention #Cardiology

Heart failure has ~50% 5-yr mortality, yet predicting decline is hard due to its non-linear course. This new #CJCO review of 20 studies across cardiology, palliative, and geriatric medicine found shared predictors of 6-month mortality: age, function and mobility, comorbidities/organ involvement, and prognostic awareness. Models are converging, suggesting unified tools could improve goals-of-care discussions and earlier palliative care. Routine IV magnesium in HF may not help—and could harm. In a population-based study, abnormal Mg levels were tied to higher mortality, and IV Mg use increased short-term death and hospitalization risks. A new #OpenAccess in #CJC suggests that reassessing practice is needed. Read here ⬇️ onlinecjc.ca/article/S0828-2… #HeartFailure #CardioReasearch

🌟 #CoolCaseOfTheMonth: Direct coaxial access via the RIJ offered a direct shot for percutaneous mechanical aspiration of a massive (37x15mm) TV/RA vegetation in CVC-associated RSIE. Under conscious sedation with continuous ICE guidance, the Penumbra Lightning Flash 2.0 16F system achieved ~80% acute debulking. Minimally invasive source control for complex right-sided endocarditis is rapidly evolving. Read here ⬇️ cjcopen.ca/article/S2589-790… #CJCO #OpenAccess #Endocarditis

🔓 For a limited time until October 25, we are offering free promotional access to a new article in #CJC! This study leverages the TriNetX research network to evaluate the clinical effectiveness of GLP 1 RAs in patients with diabetes or obesity undergoing TAVR. • At 1 year follow up, GLP 1 RA use was associated with a lower risk of multiple cardiovascular outcomes, including MACE and all-cause mortality, compared with non users. • These findings are hypothesis generating for GLP 1 RAs as adjunctive therapy, in addition to guideline directed medical therapy, in patients undergoing TAVR.
