Gastric ESD followed by defect closure using endoscopic hand suturing (EHS) with the SutuArt device.
MANTIS clip closure was unsuccessful due to very thick mucosa.
A few technical aspects, practical tips and lessons from my 2nd EHS procedure.
EHS time: 43 min 47 sec.
I would like to thank my mentor at @GI_UMCLJ dr.Luka Strniša who started EHS at our unit

What is the link between MASLD and colorectal cancer?
In this month’s #GUTPodcast, Professor Ming-Hua Zheng discusses the new international Delphi consensus statement exploring the relationship between metabolic dysfunction-associated steatotic liver disease (MASLD) and colorectal cancer (CRC). Professor Zheng is interviewed by Dr Philip Smith, Digital and Education Editor of Gut.
🔎 Key takeaways:
• MASLD is associated with an increased risk of colorectal cancer, with metabolic burden appearing to further increase that risk.
• The severity of MASLD matters — more advanced disease is associated with poorer outcomes among people who develop both MASLD and CRC.
• The relationship may extend beyond shared risk factors. The gut–liver axis and gut dysbiosis are highlighted as potentially important mechanisms linking MASLD and CRC, alongside metabolic and hormonal pathways involving leptin and adiponectin.
• The consensus highlights the importance of clinical vigilance for CRC in people with MASLD, including consideration of risk-adapted approaches to screening.
• Lifestyle-based weight loss and metabolic risk reduction are highlighted, alongside emerging approaches such as bariatric surgery and GLP-1 receptor agonists, as potential strategies to reduce risk.
The international consensus brings together expert opinion across epidemiology, pathogenesis and management of CRC in people with MASLD.

How do the best get to be the VERY best at medicine?
One of the best advanced endoscopists in the world is my friend Robert Bechara.
And he has a few traits that I think allowed him to be elite at his performance.
In this Substack, I talk about some concepts of superexpertise, how it develops, attach it to theory, and describe learnable habits that the VERY best have developed:
