BEGONIA arm 6: durvalumab + T-DXd showed encouraging 1L activity in HR−/HER2-low mTNBC, mostly PD-L1–negative: ORR 62%, mPFS 12.6 mo. But the ORR threshold was not met and ILD/pneumonitis was 20.7%!! Dato-DXd + durvalumab in BEGONIA arms 7/8 looked numerically stronger: ORR 79%, mDoR 17.6 mo, mPFS 14 mo in the all-comer cohort; ORR 82% in the PD-L1-high cohort. Of course, PD-L1 enrichment in arm 8 (and 7) likely contributed to these outcomes, so this is not definitive cross-arm proof. Still, the overall signal fits the current TNBC direction: TROP2 may be a more consistent ADC target than the fragile HER2-low label?? With ASCENT-03 and TROPION-Breast02, the PD-L1-negative / IO-ineligible 1L TNBC space is increasingly becoming a TROP2 ADC-based field. HER2-low status alone does not change that interpretation (#ASCO26) Of course, these are still phase 1/2 data. nature.com/articles/s43018-0…

🇮🇳 Practice-Changing ASCO 2026 Updates in Breast Cancer 🔹 OPTIMA • PAM50 genomics can help identify patients who may safely avoid chemotherapy 🔹 IRIS-A • Selected Stage IA HER2+ tumors (≤0.5 cm) may not require taxane-based therapy 🔹 SENOMAC • Some patients with 1–2 positive sentinel nodes can avoid completion ALND 🔹 ER-Low Analysis • ER-low (1–10%) disease may behave differently from conventional HR-positive breast cancer 🔹 PATHWAY • Tamoxifen remains a valid endocrine backbone with CDK4/6 inhibition 📌 Overall ASCO 2026 Message: • Less chemotherapy • Less surgery • More precision • More individualized treatment #ASCO2026 #BreastCancer #Oncology #MedTwitter #MVOnco

What did ASCO 2026 change? Some standards of care have now moved by years. That almost never happens. BRAF colorectal, pancreatic KRAS, and DDLPS sarcoma are three of oncology's hardest diseases SOC moved by > 1yr: ▫️BRAF mCRC: 15.1 → 30.3 months OS ▪️Pancreatic KRAS: 6.7 → 13.2 months OS 🏆 ▪️DDLPS sarcoma: 1.5 → 9.7 months PFS ⭐️ (sarcoma went from "nothing" to a win). Myeloma, GIST, HR+/HER2- breast, uveal melanoma, bladder, prostate, and RET+ lung cancer moved too. This is what decades of funded science look like when it compounds. - - - - - Source: Jori (@jori_health) 𝘗𝘭𝘰𝘵 𝘱𝘦𝘦𝘳-𝘳𝘦𝘷𝘪𝘦𝘸𝘦𝘥 𝘣𝘺 𝘦𝘪𝘨𝘩𝘵 𝘰𝘯𝘤𝘰𝘭𝘰𝘨𝘺 𝘦𝘹𝘱𝘦𝘳𝘵𝘴. 𝘛𝘩𝘢𝘯𝘬 𝘺𝘰𝘶 <3 - - - - -

If you have breast cancer you must watch this episode of @breastfriendspd featuring my incredible oncologist and friend Dr @hoperugo. She understands where we came from and where we are going, values supportive care as much as treatment, and I’m lucky to have her in my corner.
In this powerful discussion, Dr. Hope Rugo, Dr. @JAMouabbi, and Dr. @PTarantinoMD explore how ctDNA-guided therapy and the SERENA-6 trial are redefining molecular progression. 📷 Watch full episode now on YouTube - youtu.be/IGbXzLpE-LY #BreastCancer #OncologyPodcast

In 1998, the approval of trastuzumab began to change the poor prognosis of HER2-positive breast cancer for the better. Over the following nearly 30 years, anti-HER2 therapy evolved step by step: trastuzumab, pertuzumab, T-DM1, and now T-DXd. With the latest anti-HER2 ADC data, T-DXd has now been approved in two new indications for early-stage HER2-positive breast cancer. First, we changed the prognosis for the better. Now, the goal is cure...
TDXd now @US_FDA ✅ in neoadjuvant and adjuvant high risk residual HER2+ breast cancer based off DESTINYBreast05 and DESTINYBreast11: - ⬆️ pCR & iDFS w/ TDXd - Timings of RT did NOT impact the incidence or severity of ILD - New Soc for high risk disease! #bcsm #OncTwitter

T-DM1 or T-DXd after neoadjuvant therapy? Residual HER2+ disease after NAC does not automatically mean adjuvant T-DXd. 🔹 KATHERINE → any residual invasive disease qualified for T-DM1 🔹 DESTINY-Breast05 → T-DXd studied in higher-risk residual disease Key high-risk features in DESTINY-Breast05: • Initial cT4/cN2–3 disease OR • Residual nodal disease (ypN1–3) An important eligibility distinction while interpreting adjuvant escalation strategies. #BreastCancer #HER2positive #Oncology #MedEd #MVOnco

Major FDA news today for early-stage HER2+ breast cancer. T-DXd approved for two separate indications: neoadjuvant Stage II/III disease (T-DXd x4 followed by THP x4), and adjuvant treatment for residual invasive disease after neoadjuvant HER2-targeted therapy. The data are striking. DESTINY-Breast11: pCR 67.3% vs 56.3% with ddAC-THP (p=0.003). DESTINY-Breast05: 3-year IDFS 92.4% vs 83.7% with T-DM1, HR 0.47 (95% CI 0.34-0.66, p<0.0001). But the real work is just beginning. Not every high-risk patient needs T-DXd upfront. Our challenge as oncologists is figuring out who needs what. Biomarker-driven selection, ctDNA, pCR response – these questions have to follow. Escalation and de-escalation strategies needed. #bcsm #BCSM #BreastCancer
