🔬 When a T-cell lymphoma expresses CD20… think twice! An exceedingly rare case of CD20-positive primary cutaneous T-cell lymphoma, NOS, with an aggressive clinical course. The diagnostic challenge? True coexpression of CD3 and CD20—a finding that can easily raise the possibility of a B-cell lymphoma. T-cell receptor clonality and the overall clinicopathological picture were crucial in establishing the diagnosis. A striking reminder that aberrant immunophenotypes can challenge even our most familiar diagnostic algorithms. Tumors of the Breast, WHO 6th edition update: A high-grade or HER2-positive carcinoma with mucin should be called invasive ductal carcinoma/NST with mucin production/secretion Avoid “IDC with mucinous features.” Colleagues may interpret that as pure mucinous carcinoma, which this is not. Happy to share that our study is now published in Human Pathology! We found that TFE3 expression is significantly more frequent in BAP1-deficient clear cell RCCs, potentially creating a diagnostic pitfall, particularly in limited biopsy specimens. sciencedirect.com/science/ar…

Epithelioid mesothelioma in the peritoneum: • Can look morphologically similar to serous borderline tumor (pic 1) • Can show calcifications similar to low-grade serous carcinoma (pic 2) Warning: PAX8 and SOX17 are not reliable in this differential, they can be positive in up to ~15% of peritoneal mesotheliomas Helpful stains: ER and PR are almost always negative in mesotheliomas An ER/PR-positive proliferation is unlikely to be a mesothelioma Dr. Chapel. #DiagnoseThis! The patient is a 67 y/o Caucasian female with slowly progressive kidney disease and 2.3 gm/24 hr proteinuria. Aside from hypertension she has no significant past medical history. Congo red was negative, however immunofluorescence showed IgG and lambda staining within glomeruli. What is this immunoperoxidase stain? For bonus points, what was found on electron microscopy? #renalpath #kidneypath #pathology #renal
