Reassuring for anyone still hesitating on TXA at the bedside. Randomization to TXA showed no association with arterial or venous thrombotic/thromboembolic events.
❇️Best of Trauma❇️ "Thrombotic and thromboembolic events were not associated with tranexamic acid in three large randomized controlled trials" In this sub analysis of data from 3 large randomized controlled trials on TXA, no association was found between randomization to TXA and risk of arterial or venous thrombotic or thromboembolic events. journals.lww.com/jtrauma/abs…

HI-PEITHO just published in NEJM. First adequately powered RCT of catheter-directed thrombolysis vs anticoagulation alone in intermediate-high-risk PE. Here's what the data show; and what they don't. 🧵

A few things worth keeping in mind before updating practice: - No mortality benefit demonstrated; the signal is in short-term decompensation. - It remains unclear whether the ultrasound component adds anything over low-dose catheter alteplase alone.

The field now has level-1 evidence where it had none. That matters. The caveats are real, but so is the signal. @NEJMGroup @ACCinTouch #PulmonaryEmbolism #HIPEITHO #NEJM #Thrombosis #CriticalCare

(1/3) Young patient with massive PE POST thrombolysis. still in shock. HR 130 BP 78/54 Lactate 7 Cap refil > 5 seconds LVOT VTI 7. TEE shown. On norepinephrine 70 mcg/min, Vaso 2.4U/hr, ventilated. What's your hemodynamic strategy? (what we did 👇)

Interesting paper. Makes me wonder; would the results look different with partial REBOA or more precise titration strategies?
REBOA = higher mortality (47% vs 36%), more transfusions, longer time to OR. No survival benefit. Check the new study by Hatchimonji et al. bit.ly/441iuXH and Jansen's commentary bit.ly/48a2TYi. @jhatchmd @dianehaddadmd @pdowzicky @markseamonmd

The paper recommendation: PERT should be the standard of care for the management of patients with acute PE.

What we have for AI in the field of Pulmonary embolism (PE): Aidoc’s FDA-cleared AI solution for pulmonary embolism detects both acute and incidental PEs on CT scans. It flags PE on PACS/RIS. Not sure if we have it in Canada or been tested here. #Pulmonary embolism #AI

Study in Investigative Radiology (2024) showed Aidoc’s AI cut missed incidental PE on chest CT from 50% to 7.1%. Sensitivity 95%, specificity 99%. AI didn’t affect 90-day mortality but greatly improved detection
