It’s both. The Pope owns all the rights to non-profit incorporated business models, such as the 501’s. And the carrot: tax advantages.
Not-for-Profits are unincorporated, have standing on the land and are set apart from incorporated businesses. They are fully indemnified.

"Nonprofit" is a tax status, not a business model.
Many of these hospitals charge more, earn more, and pay no income or property taxes. The tax break was supposed to buy charity care for the community. Too often it buys new specialty wings and ad campaigns aimed at the best-insured patients.
Meanwhile the patient gets the bill, and the independent doctor down the street pays taxes and competes with them.
If they run like a business, tax them like one.
