Grateful for the opportunity from @atiya_dhala to give a talk today on #PEEPtitration in #ARDS at the #ISAC2026 @Methodistpccm
Here are the major take-home points:
We’ve been taught to fear #PEEP because of the RV.
But the real problem isn’t PEEP.
It’s the wrong PEEP.

I built a ventilator simulator you can learn on alone 🫁
It turns our open-access ATS Scholar #HowITeach paper into a self-guided intro taught like we teach it in the unit!
+ a 5 min asynchrony challenge!

229,410 Americans will be diagnosed with lung cancer this year. 600+ a day. 🤍
More people die of lung cancer than of colon, breast & prostate cancers combined.
Screening Day: Sat, Nov 14. Ask about low-dose CT.

Things scarier than Halloween 🎃
1. A 3 a.m. page that starts with "Just FYI…"
2. K+ 6.8. "Not hemolyzed."
3. "Can you come look? They just seem… off."
4. Your board exam date
We can only fix #4 👻

Happy #InternalMedicineDay 🩺
The best lesson IM gave me:
The answer is usually in the history.
Not the CT. Not the panel.
Ask one more question. Examine one more time.
To every IM resident & hospitalist doing that work daily: thank you.

CHEST 2026 wasn't about the posters. It was about the people.
Mentors who read every draft. Co-fellows who became family. Friends who showed up just to cheer.
And my wife, for her unconditional love and support.
Indebted to you all. 💙

HPI: 60F w/ a PMH of GPA (aka Wegener's), EGPA (aka Churg-Strauss), IgA vasculitis (aka HSP), reactive arthritis (aka Reiter's), T2DM (aka NIDDM), and PMOS (aka PCOS)…
New name for PCOS. The new name makes pathophysiological sense but, like all these name changes, will lead to more confusion than clarity. Some of us will use the old name and some the new. Notes over time will be inconsistent and confusing to providers. Patients will get lost in all the letters. I'm not sure this is a net positive change.
Wrapping up two incredible weeks on MICU @Methodistpccm 🏥 Grateful for an amazing team that made every long day worth it. Learned so much, laughed even more, and maybe spotted a 🦢 here and there. This one’s going in the memory book. 📸 #MedTwitter #MICU

I wrote this in a moment I never would have chosen. A sudden pause that made me see my life clearly.
The meaning of our work is profound. This experience simply helped me see more clearly what matters most.

Bottom line:
✅ We don’t cause RV failure by using PEEP.
✅ We cause it by using the wrong PEEP.
