#ESMO26 One of my favorite discussant. Dr @JordiRemon discusses early stage immunotherapy abstracts. Many options in the neoadjuvant and perioperative space, summarized beautifully. What is the value of adjuvant immunotherapy? Is receipt of adjuvant therapy a surrogate for fitness? #ESMOAmbassadors

Pembro monotherapy still has a role in selected pts (borderline ECOG PS or where a pt does not want to do chemo) but the control arm did not underperform. The final median PFS for Pembro mono in KN024 was only 3 weeks less than the control arm performance in PERSEE was it not?

#ESMO26 | PERSEE
We expected chemotherapy to add benefit, but the magnitude of PFS improvement even in PD-L1 ≥90% patients is striking.
Despite the limitations of subgroup analyses, these findings challenge the routine preference for ICI monotherapy in this population.
#ESMO26 | PERSEE
For patients fit enough to tolerate chemotherapy, these results strengthen my preference for pembro + CT, even with very high PD-L1.
The sustained PFS benefit and encouraging OS separation deserve attention.
Heartfelt congratulations to the investigators for conducting this academic, multicentre trial addressing a question we face every day👏

‼️ In first line treatment of advanced PD-L1 ≥50% NSCLC, PERSEE demonstrates a significant improvement of PFS with pembro-CT compared to pembro monotherapy regardless of PDL1 level of (over)expression. Practise changing trial. @myESMO @OncoAlert #ESMO26 👏👏👏 @decroisette

Academic & pragmatic phase III trial (PERSEE) #ESMO26 reshaping therapeutic choices for PD-L1 high patients (including ultra-high PD-L1 80/90%). Convincing & impressive results.
Huge congrats to the GFPC group 🇫🇷 for leading this unique and crucial study! 👏@decroisette
Presented at #ESMO26:
In a prespecified interim analysis of a phase 3 trial involving patients with extensive-stage SCLC, the median overall survival was 21.1 months with tarlatamab–durvalumab maintenance therapy and 12.7 months with durvalumab alone. Full DeLLphi-305 trial results: nej.md/4iveIgW

ALT A graph from the Original Article "Tarlatamab–Durvalumab Maintenance for Extensive-Stage Small-Cell Lung Cancer." The trial name, DeLLphi-305, is in parentheses after the title. The graph shows the overall survival (intention-to-treat population). The trial results are being presented at the European Society For Medical Oncology 2026 Congress. 🔥 TODAY COULD BE ONE OF THE DEFINING MOMENTS OF #ESMO26! 🔥
The ESMO Presidential Symposium is always one of the highlights of the congress, and today’s session promises some truly extraordinary science!
If I had to pick ONE study with the greatest potential to change clinical practice at ESMO 2026, my choice would be DeLLphi-305.
For decades, SCLC has been one of the most challenging diseases in oncology, with far too little progress and far too few therapeutic breakthroughs.
Could DeLLphi-305 finally represent the major leap forward we have been waiting for? We already know that adding tarlatamab to durvalumab as first-line maintenance significantly improves OS compared with durvalumab alone. But today, we will finally see the numbers. And I am genuinely excited to discover just how transformative this benefit might be! My personal bet for the biggest breakthrough of #ESMO26? DeLLphi-305. @StephenVLiu @sands_jacob
Of course, the magnitude of survival benefit, safety and clinical applicability will ultimately determine its impact. But the potential is enormous. Don’t miss today’s Presidential Symposium!

In ES-SCLC, tarlatamab + durvalumab maintenance significantly improved outcomes vs durvalumab alone following first-line chemotherapy + durvalumab:
OS: 21.1 vs 12.7 months (HR 0.42)
PFS: 7.4 vs 1.9 months (HR 0.34)
CRS: 49.6%; grade ≥3: 0.4%.
An impressive OS benefit! Atezolizumab + lurbinectedin maintenance also demonstrated an OS benefit in IMforte (HR 0.73). Although cross-trial comparisons are inappropriate, the magnitude of the OS benefit with tarlatamab appears numerically much greater.

🚨 ESMO 2026 presidential | DeLLphi-305:
Phase III (n=563): Tarlatamab + Durvalumab vs Durvalumab as 1L maintenance after chemo-IO.
📊 Key results:
• Median OS: 21.1 vs 12.7 months (HR 0.42) 🔥
• 12-month OS: 79.4% vs 53.3%
• Median PFS: 7.4 vs 1.9 months (HR 0.34)
• ORR: 26.4% vs 4.1%
⚠️ Grade ≥3 AEs: 46.8% vs 40.6%; CRS 49.6% (mostly low grade).
💡 58% reduction in risk of death!
Could this redefine first-line maintenance in ES-SCLC? Probably yes. Looking forward towards full data set . @OncoAlert @esmo #esmo26 #SCLC #DeLLphi305

Hard not to add the PERSEE study, (a GFPC trial 🇫🇷) so eagerly awaited for our daily practice and so often cited for its design and the anticipation of its results (1st in place for the PD-L1 high patient strategy) which are finally here and truly impressive #ESMO26 ! 📈🔥
