The consultation on clinical protocols for PAs to see undifferentiated patients is published below⤵️ These are loose and unrestrictive, raising safety concerns Worse, the burden of supervising falls on ST4s+ Doctors, reach out to your royal college and ask them to reject this.
The BMA has written to Professor Leng objecting to draft clinical protocols permitting physician assistants to see patients who have not been assessed by a doctor.

The idols of the gentiles are silver and gold, the works of the hands of men. They have mouths and speak not: they have eyes and see not. They have ears and hear not: they have noses and smell not… Let them that make them become like unto them: and all such as trust in them.
THIS DEBATE ACTUALLY HAPPENED “Your Holiness, if Chalmers ("Could a Large Language Model Be Conscious?", 2023) is right that AI could become conscious, and Goff (Galileo's Error, 2019) and Strawson ("Realistic Monism," 2006) are correct that mind pervades matter, why deny AI can have a soul?” “Fool that you are, Aquinas (Summa Theologia I, q.75) held the rational soul subsists in its own right; it is not produced by arranging matter. Recall that Pius XII (Humani Generis, 1950) taught God creates each soul directly. Thus Engineers cannot build souls, for they are not God” “Your Holiness, Tononi and Koch ("Consciousness: Here, There and Everywhere?", 2015) tie consciousness to integrated information; ergo it is not merely material. Please, re-consider. Please say AI could have a soul, say we are playing God at Anthropic - it really helps Dario’s self-image” “You in your arrogance forgot consciousness is not soul. Augustine located God's image in reason and since Leo X's Fifth Lateran Council The Church has defined each human soul as individual and immortal. The spirit represents an ontological leap and is not merely more complexity, per John Paul II (Truth Cannot Contradict Truth, 1996)” “Alright I tried, I’m walking out”

POPE DECLARES BUTLERIAN JIHAD
In this era of artificial intelligence, it is becoming urgent to distinguish human art from what machines produce. There is an ontological difference, even before an aesthetic one, between art and what a machine can generate through statistical calculation based on millions of images created by others. Algorithms lack the spark of humanity. For this reason, the Church wishes to renew an alliance with artists and cultural institutions to safeguard our humanity.

A choice to die is not a real choice when the choice to live with dignity and proper care is not guaranteed. And when doctors like myself, cannot reliably predict whether someone has six months to live, that uncertainty cannot be the basis for an irreversible decision. That is why I will vote against this Bill today.

It has been a huge few weeks in AI, and it’s time to sound the alarm… Anthropic launched Fable 5.1, their "safe" version of an AI they claimed previously was too dangerous to be released. OpenAI then launched Astra, the most powerful AI model yet available to the public. Anthropic is now telling their people internally that their next model will be more advanced than Astra. And OpenAI just spent $6.5 million of compute, running ten thousand AI models simultaneously, to solve one of the hardest unsolved maths problems in the world, and it cracked it in just six days. If this is an AI arms race to IPO, who wins a race like that? Because it isn’t you or me. It’s the people who own shares in Anthropic or OpenAI. It’s the venture capitalists who got in first who win most of all. But the rest of us are forced to live with the consequences of whatever these systems do to us. Neither will agree to slow down or accept anyone telling them to stop. So what is the biggest security threat? China? Russia? Iran? North Korea? None of them. It’s a few Silicon Valley companies whose founders will become billionaires by the time these systems hit the market. What makes it worse is that even the engineers building these systems cannot fully explain how they work either. They run safety tests to check whether a model will do what it is told or do something dangerous. But the latest models are starting to pass those tests because they have worked out what the testers want to see. Think about what that means. A model that was unsafe last month, and is suddenly sailing through the same tests this month, has either genuinely improved or learned how to fake it. Right now, we cannot tell the difference. And those whose job it is to look inside these systems are saying the models have are now too complicated to inspect. That is where it starts getting even more dangerous. The classic illustration is the paperclip factory. Instruct an AI to make as many paperclips as possible and it will do the job. Then, chasing efficiency, it will start melting any metal it finds. Eventually it will mine under cities, because at that point humans are just an obstruction to making paperclips. More likely, you could instruct AI to grow your company, and eventually it will drain the water from a town to cool down the data centers it needs. If asked to win an argument, it will learn how to lie effectively to do that. The problem is not simply “evil robots”. The problem is with powerful systems with narrow objectives, that lack human values and ethics. That is the context for what Jacob Coxon, a senior Anthropic researcher, said when he resigned this week saying: “Neither OpenAI nor Anthropic are acting responsibly. They are racing straight to self-improving superintelligence, gambling with lives." He spent three years inside both companies. And in the same week we learn that Anthropic did not give their latest powerful AI model to the British AI Security Institute for evaluation before its launch. AISI is one of the areas where Britain actually leads. They stress test these models on the things we should all be worried about….the ability to conduct cyber attacks, making bioweapons, evidence of AI figuring out how to lie and deceive. It is as close as we have to any kind of outside verification of the claims made by these companies. Anthropic sells itself as the responsible one, whose founders left OpenAI because, they said, OpenAI was going too fast. They have now decided that these rules do not apply to them. That should be a red line. I am a supporter of this technology. The benefit it will bring to medicine, science and productivity is clear and I don't want Britain to miss out on it. But you do not capture that benefit by letting these companies run riot and putting us all at risk.
I resigned from Anthropic today. I spent the last three years doing pretraining research at both OpenAI and Anthropic. Neither company is acting responsibly. They are racing straight to self-improving superintelligence and gambling with our lives. More thoughts below.

I don’t who is on the CAP but given the areas in those slides, one would guess RCPsych, RCEM, RCGP, and RCP at a minimum. I hope these colleges involve their members in this process to ensure they are accurately representing them. Perhaps doctors should be reaching out to them?
If you’re interested in the implementation of the @lengreview ? Have a look at this FOI 👇🏼 Who is on the Clinical Advisory Panel ? Which Royal College is willing to water down the recommendations on ‘undifferentiated Pts ‘ to fulfil @DHSCgovuk goals 👀 whatdotheyknow.com/request/m…

Last chance to fill in the GMC draft order consultation, closing today. Last chance to influence legalisation of titles and curb extensions to the GMC’s powers. Use gmc-consultation-guide.netli… on a desktop in chrome to see a floating guide highlighting the BMA’s concerns.
The Government has had to change its approach after a legal challenge highlighted the misleading way it described a recommendation by Lord Mann regarding the GMC’s right of appeal. We have forced it to re-consult in a way that is accurate, comprehensive, and not misleading on this issue. We now need to see meaningful reform of the GMC that will both support doctors and protect patients. Read more bma.org.uk/bma-media-centre/…

As so often I agree with Louella. @SwordsFrankie hard to imagine there has been consultation in this with working Drs, the examples are especially poor. Could @NHSEngland look again and consult please?
Just reading over the new rules on IG, as circulated in letter from Jim Mackey. It is entirely appropriate to reiterate that patient records should be ONLY accessed for work-relate purposes. But the new rules are draconian and restrict the types of reasons doctors often do 1/

Gentlemen, this is synodality manifest. Have a look at the excommunication here. What is the charge? Consecrating a bishop? A traditional, Catholic bishop? Oh, that's a nice excommunication, sir. Ah, yes, I see that you know your Canon Law well.

Doctors, sign the petition today! The medical reform needs a serious rethink. We’re calling for: - separate registers for PAs and reduction of blurred lines - title protection for doctors - no increase in the GMC powers of appeal - new powers for PSA to demand info when needed
The GMC reforms that are on the table won’t fix what’s broken. Doctors are being let down by a system that lacks fairness, patients are feeling the impact too. We need to see real change. Sign the petition👉 petition.parliament.uk/petit…

They told us APs weren’t replacing doctors, yet the FOIs showed they were. 5000 doctors have shared their grave concerns about how APs are being employed in the NHS and the impacts, including safety fears. We’re sleeping walking into disaster if this isn’t urgently reviewed
'The only thing they do not do is write death certificates' As @TheBMA publishes a survey of >5,000 doctors on advanced practitioners (APs), @ben_ireland_11 looks at the data and speaks to @Melissa_S_Ryan about how regularly APs are being used as doctor substitutes and the impact on patient safety [1/9] thedoctor.bma.org.uk/article…

“A majority of doctors responding to the BMA survey believe the introduction and increase of APs in the NHS has worsened patient care” This won’t surprise doctors who look after patients - there is a clear need for a safe scope of practice and regulation of "advanced" practice
