Another benefit for physiologic pacing - better assessment of underlying coronary pathology in acute MI: 2 different patients, both with physiologic RV leads presenting with inferior STEMIs allowed earlier identification by our #interventional colleagues and faster PCI. #EPeeps

EP findings supported S-F AVNRT with an upper common pathway. Interestingly, this phenomenon emerged after antegrade SP ablation. Although antegrade SP conduction was completely abolished during AP, AVNRT remained inducible by VPS.

LAAC vs best medical care
Thrombotic events - higher
Bleeding events -- higher (counting procedure; also see AVERROES)
CV death -- higher (please don't dare tell me that it was noise because if you do, let's relook at PROTECT and PREVAIL
LAAC done in experienced labs

We can't always map during VT due to hemodynamic instability or non-inducibility, but when we can, the results can be truly amazing with a good chance of the full circuit being revealed! šŖ Case performed by @Maeskander #EPeeps

Measure twice and cut once š! Retrograde access and one-burn ā¤ļøāš„ elimination of this bigeminal PVC from superior mitral annulus. Notice the CS activation bracketing at CS 5,6 which was just across from the earliest endo location. Case performed by "One-burn" @Maeskander. #EPeeps

Do I have this right?
- ABIM requires yearly fees from physicians who are already cert to āmaintain certificationā
- no evidence that MOC helps pt care
- ABIM will report you as ānot certifiedā if you donāt pay
- hospitals require certification
Idk kinda sounds like extortion
Nope. Not happening until I find out what this gets me , @ABIMFoundation. ā© What are we paying for? Iām certifiedā¦I took my boards and passed them.

What is it about the R-L interleaflet triangle (ILT) between the right and left coronary cusps that causes it to be the site of successful ablation for so many LV summit PVCs? @Maeskander @DrRoderickTung @DrFerminGarcia @javadm20 @narrowQRS @B_Naz_MD @MBarakatMD #EPeeps #ablateVT
