#ICYMI: the long-awaited updated @ASH_hematology #ITP guidelines are here! With so many new agents for managing ITP, I’m excited for these recs to better help our patients achieve durable platelet responses and improved QOL Link: ashpublications.org/bloodadv…

We talk about bsAb revolution coming across the field of oncology - now more of it in classical hematology as well! Letter in big @TheLancet: Time-limited teclistamab in dara-refractory ITP with basically instant platelet normalization 👏 #MMsb bsAbs to the rescue!

Simple concept of GN. It’s all about location! 1. Subendothelial deposits/injury by Ig, immune-complexes, complement, other mechanism leads to influx of leukocytes= inflammation=GN 2. Subepithelial deposits/injury= protected by GBM & endothelial cells=no inflammation=No GN. 1/3

Our first RCT on using an LLM on diagnostic reasoning is out! And the results are 🔥🌶️... adding ChatGPT did NOT improve diagnostic accuracy or reasoning, and the AI alone outperformed ALL the humans. What does this mean? A 🧵⬇️ xitter.gq/EricTopol/status/18509…
Replying to @EricTopol
A small randomized trial of generative #AI for diagnosis again (as seen in a few previous studies) shows higher performance for #AI than physicians + AI. May indicate that physicians need to be trained on how to incorporate AI. jamanetwork.com/journals/jam…

1/ Challenge accepted. I have run a series of breast mammograms (x-rays), ultrasounds and MRIs through versions of GPT4 and posted the results here previously. Even though it's data contamination to repost images I already used on X, it's an easy bench mark. So how did Grok do on breast radiology? A little better than GPT4v, but not a single diagnosis correct. 👇
Try submitting x-ray, PET, MRI or other medical images to Grok for analysis. This is still early stage, but it is already quite accurate and will become extremely good. Let us know where Grok gets it right or needs work.

Hey #NeuroTwitter , check out my #neuromuscularbuckets. It's a method to work backward when evaluating patients with neuromuscular disorders. Sometimes you know the patient has a peripheral nerve disorder, but you get stuck on where to go next. That's where the buckets are helpful. Prefilled buckets that localize to one of the six main aspects of the PNS can help you think of diseases/entities that you don't want to miss. If you have a clinical picture of those disease in your mind while thinking through the case, you'll check for certain things you may have otherwise skipped over! A thread... #medsocial30 @NDECinstitute Day 12

1. Which prophylaxis do you chose after a hip or knee surgery? Simplistic: ASA=LMWH Correct: ASA has similar mortality & PE as LMWH but 1-2% more uncomplicated DVTs In people at very high VTE risk, may consider LMWH if convenience & cost not an issue xitter.gq/AnilMakam/status/16161…
New @NEJM pragmatic trial says aspirin is same vs LMWH after fracture Last summer a JAMA cluster RCT study said aspirin was worse after hip/knee replacement In reality both say the same thing LMWH has similar mortality but 1-2% less VTE within 90 days nejm.org/doi/full/10.1056/NE…
