Watching Surgeons at the Edge of Life on BBC2. A senior surgical care practitioner described as "originally a nurse, who trained to be a type of surgeon". Hmmm...... Talks of "a whole gambit of things that goes through your mind."... Clearly not the brightest!

Owen Dickinson , the nurse consultant who KILLED 7 PATIENTS and harmed 58 , has still not been struck off the @nmcnews register nor charged by the police . Read the Trust board papers below and tell me if this is ‘ regulation’ of Advanced practice 👇🏼 therotherhamft.nhs.uk/sites/…
When your negligence and misrepresentation has harmed and killed NHS patients: Go and work 'independently' in aesthetics. Have your nugatory interim conditions lessened in April; Maintained in September. That's how we do 'advanced practice' In the nursing profession.

Doctor substitution is being championed at the highest levels yet anyone with a grain of common sense or integrity knows it’s nonsense I wonder what platitudes @theRCN or @RCEMpresident would offer these parents given their involvement in enabling this scenario?

Heartbreaking for the parents to go through this. They have showed nothing but grace and dignity in the most unimaginable circumstances. Their thoughts that every patient should be reviewed by a senior doctor in ED if referred by GP is not only sensible but should be the standard
Replying to @JanetEastham
His parents are calling for every child referred to A&E by a GP to get a documented examination by a senior doctor before being discharged. Dylan's mum Corinne said: “We just want to keep children safe. It’s too late for us, but it’s not too late for others."

You would think that when a GP does a referral to the hospital the patient is seen by a doctor But the NHS is replacing doctors to the detriment of patients with nurses/ANPs/PAs/pharmacists/ACPs For this young child and his parents that didn’t end well There’s more and more people coming to harm in the NHS due to this replacement - why are the public so silent? @andyburnham

Dylan Cope, 9, died of a ruptured appendix & sepsis after a nurse practitioner (parents thought she was a dr) diagnosed flu GP had flagged possible appendicitis Key symptoms omitted from discharge notes Nurse lied at inquest blaming Dr Coroner ruled neglect.
Dylan Cope died after an A&E nurse practitioner mistook his abdominal pain for flu, having ignored his GP’s referral for suspected appendicitis. His family thought she was a doctor. A coroner found he would have survived had a senior doctor examined him. telegraph.co.uk/news/2026/10…

Nine year old Dylan Cope died from sepsis. His GP had suspected appendicitis and sent him to hospital. A paediatric nurse practitioner assessed him in A&E without reading the GP’s referral. His abdominal pain was attributed to flu and he was discharged without a documented examination by a doctor. The coroner concluded that his death could have been avoided. Dylan’s family have suffered an unimaginable loss. As a doctor working in general practice, what stands out to me is that his GP had recognised the possibility of appendicitis and referred him for further assessment. That concern needed to be heard and acted on. When we refer a patient with a suspected serious illness, we need to know that the concern prompting that referral will be properly assessed. A different diagnosis may emerge, but the referral’s findings must be considered before a patient is sent home. Dylan’s father told the inquest that he believed the practitioner assessing his son was a doctor. He also repeatedly raised concerns about appendicitis. Families deserve clarity about who is treating their child and to have their concerns heard. This case raises serious questions about how referrals are handled and when an examination by a doctor must take place. Employers must ensure that supervision arrangements translate into the assessment a patient actually needs. There is also a wider discussion about advanced practitioner roles. The BMA recognises their valuable contribution within healthcare teams. It calls for nationally agreed limits of practice and clear supervision requirements, with advanced practitioners supporting medical teams rather than replacing doctors on clinical rotas. Its position is that they should not independently diagnose or manage patients whose underlying condition has not yet been established. I support those safeguards. Clear responsibilities protect patients and support staff to practise safely. Dylan’s parents are asking for children referred to A&E by a GP with suspected serious illness to receive a documented, face to face examination by a senior doctor before discharge. That is a call we should take seriously. They should not have had to lose their son to ask for it. telegraph.co.uk/news/2026/10…

I have a suggestion for @NHSE_WTE @NHSEngland @RCollEM It helps all- including the public. And is about being transparent. Open the Emergency Medicine training posts to any member of staff. I can’t see the point of having training numbers in Emergency Medicine if this is what a non- doctor can do It’s clearly deemed to be safe - in which case- let all train together @DHSCgovuk And stop suggesting only medical people should do training posts- maybe @DacreJane can pick this up in the review. Then again- competition ratios @TheBMA It’s either safe. Or it’s not safe. It’s either a specific training needed Or people can be “differently trained” to do same thing. Make your mind up. Rather than these situations. For the record? It shouldn’t. And I strongly believe this is unfair on the public whose care & safety we should provide to the highest order possible. But it’s happening and being pushed by all. I mean even @gmcuk wants to hand CCT to non- medics I think. So open the doors and go for it. Be transparent- clearly not enough doctors want to do this speciality so we need other staff to help. Or just admit you have lost control of workforce and training. Thanks for considering.

Revealed: the unsafe doctor substitutions putting patients at risk. We need NHS leadership to wake up. Hospitals knowingly using doctor substitutes, following years of shambolic workforce planning by successive governments, is a serious threat to patient safety. We are calling for a prohibition on the use of non-doctors on doctors’ rotas and an urgent review of advanced practitioners’ scopes of practice. Only doctors should be able to carry out work that requires a doctor’s unique training, skills and expertise. Read the full story: thedoctor.bma.org.uk/article…

I am less qualified than a podiatrist to provide foot care Or an ITU nurse to provide ITU nursing That’s a fact. It’s also called working within training, scope and to regulation If I do work as a podiatrist or an ITU nurse - I will cause harm Simple When you think of it

‘They do the same medical training ‘ This is what AHP ACPs believe. This is a lie @ScouseAHPLeader .
In 2021 @bealnamulla1 said: ‘All ACPs will be credentialed to ST3 level because they do the same medical training, then post credentialing to consultant level’ She told consultants not to be worried for their jobs A new paper which she co-authored proves that this was a lie
