One of the most highly anticipated Hot Line sessions at #ESCCongress 2026 delivered an important result.
Congratulations to my dear friend @MasriAhmadMD and the entire ACACIA-HCM investigator group and co-authors.
ACACIA-HCM is the first positive phase 3 trial in symptomatic nonobstructive HCM. 🧵👇
And the benefit extended beyond the primary endpoints.
Aficamten also produced statistically significant improvements in:
• NYHA functional class
• Exercise performance
• NT-proBNP
Overall, the signal was consistent across several clinically meaningful measures.
Safety remains central when using cardiac myosin inhibitors.
Aficamten was generally well tolerated.
Most episodes of LVEF <50% were managed with simple dose reduction, an important practical observation from the trial.
The bigger message:
For the first time, we now have a positive phase 3 trial of a targeted therapy in symptomatic nonobstructive HCM.
That potentially opens a new therapeutic chapter for a phenotype where treatment has historically focused largely on symptom management.
Longer-term follow-up will be important.
The ongoing FOREST-HCM extension will provide further efficacy and safety data, with >80% of eligible ACACIA-HCM participants already enrolled, according to the presentation.
Congratulations again to Dr. Masri and the entire ACACIA-HCM team.

We presented the ACACIA-HCM trial. A milestone for us and our patients, having a therapy that influences many important clinical domains that are relevant to patients and physicians alike.
This is the start in nHCM. An impactful one.

Across 12 Hot Line sessions, ESC is presenting 59 pivotal trials spanning heart failure, coronary syndromes, prevention, AF, structural intervention, devices, imaging, digital health and more.

🚨 5th Universal Definition of Myocardial Infarction ( major change in MI classification ) 🚨
The old numerical system is now updated.
MI is now classified into 3 clinical categories based on mechanism:
• Primary MI
• Secondary MI
• Procedure-related MI

4. Procedure related MI is redefined
The old PCI/CABG-specific biomarker thresholds are removed.
Diagnosis now focuses on a true procedural coronary complication + objective myocardial injury/imaging evidence, applied across cardiac procedures.
My main takeaway:
The 5th UDMI moves us away from simply asking:
“What MI type is this?”
and toward:
“What is the mechanism causing myocardial injury and ischaemia?”
A more clinically intuitive and pathophysiology driven framework.
