CAAN-AF Trial #CHRD @adelaide87018
Our previous meta-analysis of observational studies suggested CRT patients with AF had a significant 58% reduction in all-cause mortality with AV node ablation - jacc.org/doi/10.1016/j.jacc.…
We designed the CAAN AF Trial supported by the @nhmrc and critical industry partners - in order of support @bostonsci @AbbottNews @Medtronic @MicroPort - Methodology paper - heartrhythmopen.com/article/…

RCT showing no benefit in the routine use of AV node ablation

‼️ Now online in the #EHJ @ESC_Journals: The Vasc-PFA study

@ehj_ed
Our study provides important insights into the acute coronary vascular effects of monopolar biphasic #PulsedFieldAblation
🔬 Key findings:
👉 Acute, localized endothelial dysfunction occurs following coronary exposure to PFA, while vascular smooth muscle function remains preserved.
👉 The effects are site-dependent: endothelial dysfunction was least pronounced after cavotricuspid isthmus (CTI) ablation and most evident following great cardiac vein (GCV) and epicardial PFA.
👉 Selective endothelial injury with preserved smooth muscle cell function may explain the observed vasomotor dysfunction and why nitroglycerin can rapidly reverse clinically apparent coronary spasm despite persistent endothelial impairment.
📌 Why does this matter?
Anatomical factors may influence electric field delivery and the degree of coronary exposure. The findings also raise important questions about the potential relationship between acute endothelial injury and delayed coronary complications, including myocardial ischaemia, ventricular arrhythmias, and longer-term vascular remodelling.

🎯Discussion:
➡️The proposed mechanism is plausible: balloon-in-basket geometry may provide more uniform circumferential tissue contact and electric-field delivery, while pentaspline PFA depends more on apposition, rotation and lesion overlap.
➡️Previous data - Pentaspline PFA delivers targeted tissue transfer efficiency of only 2.7%. (loss to blood flow) vs. 12% with balloon-in-basket PFA
pmc.ncbi.nlm.nih.gov/article…
🤔Caution:
➡️The durability comparison rests on only 17 redo pts.
🧑🎓Povocative data, and the biological hypothesis deserves prospective testing. Platform effect, technique effect—or both?
So, if you would like to redo the poll -

🔯Results
➡️PV reconnection
🔴 Farapulse: 28.1% (9/32) vs. Volt: 2.9% (1/35) (p=.003)
➡️All PVs durably isolated/patient
🔴 Farapulse: 37.5% vs. Volt: 88.9% (p=.049)
➡️Farapulse failures clustered in the inferior veins (7/9)
➡️Clinical Recurrence, however, was equivalent:
🔴 Farapulse 17.6% vs. 🔵 Volt 20.6% (p=0.1)
The disconnect between electrophysiologic durability and clinical recurrence could be explained by methodology (symptomatic recurrence in FP vs. protocol-driven monitoring in Volt).

🔯Study Details:
➡️Retrospective, 2-center study of 132 consecutive pts. undergoing de novo PFA for symptomatic AF: Farapulse (n=86) and Volt (n=46).
➡️Farapulse - 53.5% Paroxysmal
➡️Volt - 60.9% Paroxysmal
➡️100% acute PVI.
➡️ Recurrence at median 14.3 months - Farapulse (10.5%), Volt (19.6%)
➡️ Redo ablation - 8 Farapulse and 9 Volt.
✴️These were not equivalent cohorts. Volt was trial-protocol-derived, whereas Farapulse represented routine real-world practice, with ⬆️ HF burden & ⬇️ LVEF.
➡️Propensity matching (34 pairs) balanced baseline characteristics for comparison of clinical recurrence—but could not be matched for the durability analysis.

✴️An interesting challenge to our assumptions.
➡️ Question - Are PFA systems comparable for
long-term PVI durability after equivalent acute PVI?.
➡️Let's start with a poll for #EPEEPS
✴️ Which PFA platform do you currently favor for AF ablation?

Back from the 21st Annual International @VTSymposium in New York. Thank you to the course directors Frank Marchlinski, Dave Callans, @Davilandre , Vivek Reddy and Srini Dukkipati for the invitation. Grateful to have been there as faculty and to share our experience with subcutaneous, extravascular and parasternal ICDs, including our workflows for safe, efficient and innovative implants, such as video-assisted access to the retrosternal space.
It was also an honor to present IMPACT-CTI as one of three finalists in the Late-Breaking Clinical Trial session, asking whether energy source impacts the chronic effect on the coronary artery when ablating at the cavotricuspid isthmus. Simultaneously published in JACC:Clinical Electrophysiology.
Thank you to my co-authors and to the @ARGJonesboro team in Jonesboro.
And the best part, as always: seeing so many friends. As @NeuzilPetr put it, the elegant VT Club. 🫀
