Closing in on the VExUS Doppler Masterclass in Toronto on September 28!
It's an awesome opportunity for an in-depth, experience with VExUS and hemodynamics for any resuscitationist

Best tip I ever received for placing femoral lines during CPR was at #HR2023 conference 👇
Assign someone to manually stabilize the pelvis while you are accessing the groin (from opposite side of bed) - doing this dramatically reduces the movement of the leg & vessels as you are accessing 🔥🩸

Can I start a thread about challenging femoral access please?
Intensivists, interventional radiologists, cardiologists- Share your top tips please.
I remember a podcast with Deidre Murphy talking about “deep pelvic kits”- is this true?

(6/6) Myth 5: I just use [x test] to tell if my patient is dry
Insert the following as [x test]: Swan, PPV, IVC, POCUS, VEXUS, etc.
Response: Fluid management requires integration of perfusion assessment (microcirculation), fluid responsiveness, and fluid tolerance assessments

Thanks @ReardonPM and looking forward to future collaborations!
Thanks to @ReardonPM for inviting @john_basmaji and myself to give @UTSC @SHNcares Department of Medicine Grand rounds on venous congestion: where evidence meets bedside physiology.
Key take homes👇
1) CVP is insufficient to detect organ level congestion
2) Congestion is NOT volume overload ❌ - it depends on the interplay between a ton of factors (RV, LV, pericardium, pleural space, intrabdominal pressures and more!)
3) #VEXUS is feasible for clinicians with focused training ✅
4) #VEXUS is associated with organ dysfunction - whether it is modifiable is still a question 🤔
5) We are trying to solve this with the Andromeda-VEXUS study (and more!) - stay tuned as this is an area of active research 🌎
Thanks for having us and I will post the youtube link to the lecture when it is available!

In patients with difficult parasternal windows, especially COPD, subcostal short axis is commonly a rescue. In this patient, note partial fusion of RCC and LCC, as well as good delineation of the pulmonic valve

1/ 44 yo patient came to ED with SBP 70 mmHg, HR 150-160 atrial fib. Was s/p ablation for atrial fib 12 days ago and had sudden presyncope and palpitations. POCUS: plethoric IVC and reverse S on HV,diffuse B-lines, TRpv 35 mmHg (so sPAP about 55-60), & the following other images:

When B-lines are coalescent, consider the percentage of the image occupied by B-lines (ie, the percentage of white screen compared with black screen below the
pleural line) and then divide by 10 (ie, 70% of
“B-lines hyperechogenicity” is about 7 B-lines)
