Drastic case with open surgical pericardiotomy and cardiac massage in cathlab today. Late presentation STEMI that got TNK then rescue PCI. Acute rupture with reperfusion injury into mushy ventricle. Won’t have happened with early primary PCI. Reminder to not delay seeking care.

Real situation @StJohnHealthOK. #isadoctorinhouse? I have a nice patient in his 60s with a bio prosthetic Medtronic Mosaic #25 placed in 2008. He comes back in with similar symptoms (SOB) and clean coronaries. Gradient on TTE and TEE are in mid-teens and gradient on cath is 23
BSA is 2.12 and his indexed EOA is 0.7. He’s a commercial truck driver and can’t drive until his valve is cleared. His lungs and PFTs are fine and his Hgb is 15. Why is he Sob and what should I do next? #Helprequested. @TCTMD @SCAI Any thoughts or follow up questions are welcome
