🔥Teaching point :
Transpancreatic sphincterotomy, quick points:
Wire in the PD, leave it there — that's your landmark. Sphincterotome in, cut the septum toward 11 o'clock. Keep the cuts short. The septum's thinner than you think.
Bile duct shows up at the top of the cut. Can't see it? Poke around the apex, don't just keep cutting.
Drop a PD stent and give rectal NSAIDs. Every single time.

🔥Some weeks stay with you.
This was one of them.
Our third-year GI fellows are only a few months away from completing their fellowship, and this week I had the pleasure of watching Dr.Mohammed Kawaja and Dr. @DAlshreiif Alsharif successfully perform selective bile duct cannulation during ERCP using my favorite approach: direct cannulation with a sphincterotome.
People often think teaching is about passing on techniques. That's part of it. But it's not the heart of it.
To me, teaching is watching someone struggle, improve, gain confidence, and eventually make the procedure look effortless.
I've always had one simple rule:
Show me your interest, and I'll invest everything I have in teaching you.
Curiosity can't be taught.
The videos below are only 60 seconds long.
What they don't show are the months behind those 60 seconds. Early mornings, difficult cases, countless questions, small corrections, and steady progress.
Seeing that growth is the greatest reward of being a teacher.
Proud of both of you. The best is still ahead.
Direct Cannulation with a Sphincterotome.

Fortunate to learn from and work with an exceptionally dedicated and collaborative research team! 💫
🧬 Scholarly Sunday
Dr. Mills and team are advancing how we detect pancreatic cancer spread. Their study shows that looking for tiny traces of cancer DNA in abdominal fluid can help identify disease earlier and more accurately than traditional methods.
This could lead to better treatment decisions and improved outcomes. 👏

Today I learned from the great Marty Freeman (@UMN_GI) that he uses tapered tip catheter for epi injection during ERCP for post-sphincterotomy bleeding:
1) Easier to get over elevator vs needle
2) Less likely to cause mucosal tear/scrape
3) Allows for effective SM injection

Sessile serrated lesions have lower dysplastic potential than tubular adenomas yet are THE LEADING CAUSE of post-colonoscopy colon cancer because they’re so frequently missed.
Adherent “mucus” in a well-prepped colon is a MAJOR clue. Resist the urge to wash it & look closer first.

Maybe “earlier” isnot always “better”.
