In our study of outpatients with MDS, bleeding was not predicted by presenting platelet count, TXA use, platelet transfusion or anticoagulant/antiplatelet therapy, and 11% still developed major bleeding, which occurred at a wide range of platelet counts

There were 734 near miss wrong blood in tube (WBIT) errors reported to SHOT in 2021. WBIT can lead to ABO incompatible transfusion.
SHOT have developed a WBIT investigation form to help identify barriers and human factors that may contribute.
