A high A1C isn't always about sugar. In 50 non-diabetic adults with iron deficiency anemia, mean A1C was 7.4% vs 5.9% in matched healthy controls. After 3 months of iron it fell to 6.2%. Lab numbers read best in context.

1,961 adults, 68 weeks, once-weekly semaglutide 2.4 mg or placebo, both with lifestyle support: Average weight change was -14.9% on semaglutide vs -2.4% on placebo in the NEJM STEP 1 trial. Half the semaglutide group (50.5%) lost 15% or more of their body weight, vs 4.9% on placebo, and physical functioning scores improved more. Fine print: adults without diabetes; the most common side effects were nausea and diarrhea, typically mild to moderate and fading with time. Questions about GLP-1 care? 94 people, 47 couples, one argument staged in a lab: Couples given oxytocin by nasal spray before a structured conflict discussion showed more positive communication relative to negative behavior, and lower cortisol (a stress hormone) afterward, than couples given placebo. Double-blind, published in Biological Psychiatry. Fine print: one small lab study, a single dose, measured during one conversation. Ever notice how closeness changes the tone of a disagreement? 790 men aged 65+ with low testosterone and symptoms, 1 year of testosterone gel or placebo: Testosterone significantly increased sexual activity, sexual desire and erectile function in this NEJM report from the NIH-funded Testosterone Trials. Men on testosterone also reported slightly better mood and fewer depressive symptoms, and adverse event rates were similar in both groups. Fine print: no change in vitality (fatigue) scores, and these were men 65+ with confirmed low levels. Wondering where your levels sit? 36 randomized trials, 8,480 women: testosterone improved sexual desire, arousal, orgasm and pleasure in postmenopausal women vs placebo or a comparator. This Lancet Diabetes & Endocrinology meta-analysis also found less sexual distress, with no serious adverse events recorded. Fine print: the benefit applies to postmenopausal women with low desire that causes distress, and the authors preferred non-oral forms like creams or patches, which kept lipids neutral. 218 randomized trials, 14,170 people with depression: walking or jogging, yoga and strength training each beat active controls like usual care. Benefits rose with intensity. Caveat: most trials carried a high risk of bias. Mood is a body system too. BMJ, 2024

172 women aged 45-60, none depressed at the start,, 12-month RCT: 32.3% on placebo developed clinically significant depressive symptoms vs 17.3% on transdermal estradiol plus micronized progesterone. The gap showed up in early perimenopause, not later. JAMA Psychiatry, 2018

27 randomized trials, 1,890 men: testosterone treatment was linked to a significant drop in depressive symptoms vs placebo. In this JAMA Psychiatry meta-analysis, men on testosterone had 2.3x the odds of a 50%+ symptom reduction (OR 2.30). Dropout rates showed no significant difference from placebo. Fine print: the trials varied in design, so the authors call for more trials with depression as the main endpoint. Mood and energy questions? 2,362 women, tracked for 4 years: in late perimenopause, processing-speed scores stopped improving with practice, while women before and after the transition kept improving. Verbal memory showed the same pause through early and late perimenopause. That's SWAN, published in Neurology. The encouraging part: after menopause, learning rebounded to premenopausal levels. The authors say this fits the memory trouble women report, and that it may be time-limited. Questions about perimenopause? Reply, and follow for the research. doi.org/10.1212/WNL.0b013e31…

ED isn't only a bedroom issue. In 9,457 men aged 55+ followed in a JAMA study, ED was linked to a 1.45x risk of later cardiovascular events. The authors put that in the range of current smoking or a family history of heart attack. Worth mentioning at your next checkup. Agree?
