These Kaplan-Meier curves finally separateโ€ฆ by about the width of a pixel... The curves ride on top of each other for years before a modest late subatomic divergence, and then the blue arm takes a dramatic vertical plunge at the far righ, right about the time the numbers at risk collapse to single digits. Truly a triumph of statistical significance over clinical drama. Novartis is already calling a ~20% relative reduction โ€œclinically meaningful.โ€ Somewhere a health economist is quietly calculating the cost per life-year for treating 5,100 patients to avert ~27 deaths.@5_utr

๐Ÿ›ก๏ธThe First Personalised mRNA Vaccine Shows Spectacular Results in Melanoma. Phase 3 INTerpath-001 trial evaluated the first-in-class personalised mRNA vaccine Intismeran autogene (mRNA-4157/V940) combined with Pembrolizumab in resected high-risk Stage IIB-IV melanoma. ๐Ÿ“ŠRFS at 24 months was 82.2% vs 70.0% with pembrolizumab alone (HR 0.58; p<0.001). ๐Ÿ“Š42% reduction in the risk of recurrence or death. ๐Ÿ’INTerpath-001 sets a new benchmark for adjuvant therapy in melanoma. We are moving from "one-size-fits-all" treatments to individualised, bespoke immunotherapy

There is an allegation that a journal editor demanded a $300K bribe to publish a manuscript. This editor used to work at Science and now is editor in chief of a top cancer journal. There is no doubt a very shady business that surrounds academic publishing, sustained by rent seekers throughout the biomedical system. Institutions have attached enormous rewards to getting through a journalโ€™s gate while paying far less attention to who controls that gate and how the decisions are made. A major problem in academic research is how much judgment has been outsourced to journal names and the editors who control access to them. Professional journal editors are former scientists who left research for a more straightforward job, often paying far less than a faculty position or a job in industry. Yet universities and scientific organizations have given these editors tremendous power over careers and the allocation of hundreds of billions of dollars. As the biomedical system expanded and became more compartmentalized, there were too many scientists and laboratories for people evaluating them to assess everyoneโ€™s work themselves. The journalโ€™s name became a substitute for judging the science. Where something was published determined what it was worth. That judgment carries enormous financial consequences. Academic rank and stature depend on it. Venture capitalists use the journalโ€™s name to reduce the perceived risk of investments. Pharma directs its attention toward those publications. Universities use them to select people for administrative positions, some carrying seven-figure salaries. Having your name on the right papers can open those doors even if the papers were completely wrong to begin with. A Cancer Cell paper can be worth far more than $300K. An author can use it to raise many times that amount from investors and start a biotech company. The financial value attached to the publications a single editor decides to accept could approach a billion dollars while that editor makes $80K. Meanwhile, almost no attention is paid to how qualified these individuals are to make such consequential scientific assessments, why they have been given so much authority, or why their compensation is so low relative to the financial interests surrounding their decisions. That disparity creates opportunities for impropriety. However, this remains an allegation, and the editor is innocent until proven guilty.

Great to share some thoughts on โ€œMentorship 101โ€ at the #AILCC26 ๐Ÿ‡ฎ๐Ÿ‡ช One concept I wanted to highlight: hedonic adaptation or โ€œbecoming accustomed to successโ€ Yesterdayโ€™s dream becomes todayโ€™s normality and tomorrowโ€™s expectations. In medicine, we constantly chase the next publication, grant, promotion or achievement. But do we ever stop to appreciate how far weโ€™ve come? We guide a d help our mentees on how to be successful. How about guiding them on how to feel successful? @DrJNaidoo @FordePatrick @gerryhanna #Mentorship #MedicalEducation #Leadership

How should we govern AI in health professions education? 1/5 AI in health professions education is not just a technology question. It is a question of governance, ethics, academic integrity, equity and professional identity. Our critical narrative review examines these challenges. ๐Ÿ“„ shorturl.at/V450L 2/5 First: governance. Institutional responses may be fragmented, while educational applications of AI continue to evolve. How can policies remain useful without becoming quickly outdated? 3/5 Second: ethics. AI raises difficult questions about autonomy, beneficence, non-maleficence and justiceโ€”particularly when systems are opaque or data-driven. 4/5 Third and fourth: generative AI challenges established ideas of authorship and academic integrity, while bias and unequal access raise concerns about equity and professional development. 5/5 Responsible AI in medical education needs more than a blanket โ€œallowโ€ or โ€œban.โ€ Our review proposes a risk-based governance taxonomy. What should institutions prioritise first: transparency, assessment integrity, fairness or accountability? #MedicalEducation #AIethics
