❤️🔥 A Young Woman, a Critical Left Main, and a Bailout That Worked ❤️🔥
🩺 The Presentation: A young female was admitted with Class IIIB #angina 🚨. Her #coronary #angiogram showed critical, technically difficult ostial #LeftMain disease, compounded by critical #ostial #LAD and #LCx lesions, with #disease at the most important checkpoints of the #coronarycirculation 🛣️.
🔌 Access Challenge: Via the #femoral route, a 7Fr #JL catheter could not selectively engage the ostial Left Main, so the procedure proceeded with the catheter hanging at the #ostium 🎣.
🧵 Wiring: A workhorse wire was manipulated with some difficulty across the diseased ostial LCx and parked in the distal LCx. A second workhorse wire was similarly manipulated across the Left Main into the ostial LAD and parked in the #distal LAD ✅.
🎈 Dilatation and #Complication: After pre-dilatation of the LCx, #kissingballoon dilatation was performed to open the ostial LCx, ostial LAD and Left Main. But the left #injection angiogram showed diffuse dissection involving the Left Main and LAD ⚠️.
🔩 #Stenting: A #DES was positioned in the LCx and an #NC balloon in the LAD. The first DES was deployed in the LCx, with its #proximal 1-2 mm kept in the Left Main. The angiogram then showed a dire picture: no flow in the LAD due to a #dissection flap 🆘.
💪 The #Bailout: The proximal #struts of the LCx stent were #crushed with the NC balloon, and a second DES was successfully deployed from the Left Main into the LAD 🎯.
✨ #Optimization: After the struts in front of the LCx were opened, both the LAD and LCx were post-dilated, and a final proximal optimization technique (#POT) in the Left Main restored #TIMI III flow on the final angiogram 🏆💓.
