🚀 Exciting work by @brian_rini and colleagues! IRIS-RCC brings us closer to reproducible, individual-sample molecular subtyping in RCC. We are exploring a complementary approach in mRCC: vascular architecture as a biomarker of IO sensitivity, assessed on routine H&E or CD31 IHC. Our initial data suggest that low vessel branching may enrich for IO response, while our vascular score has also shown baseline prognostic value in CheckMate 914. The exciting opportunity: a potentially scalable, pathology-based biomarker that could be easier to integrate into routine clinical workflows. Next step: validation in digitized H&E from large, randomized mRCC cohorts—critically, to distinguish predictive from prognostic value. Several groups have attempted to predict the IM151 RCC clusters with flawed methods. We developed IRIS-RCC (Individual RNA-seq Intrinsic Subtyping for RCC; available at irisrcc.vindhyadatascience.c…), to predict clusters using an individual-sample model. nature.com/articles/s41698-0…

Interesting data from Phase 2 study #COMRADE at #ESMO26: Early on-treatment #ctDNA may help identify patients at risk of progression after just one cycle of #radium223. Possibly one more step toward biomarker-guided, adaptive radiopharmaceutical therapy. We are getting more and more evidence about a potential key prognostic role of ctDNA also in #ProstateCancer including evidence that it may be used as a guide for treatment modulation - it will be most important now to prospectively include ctDNA in more trials and further validate it #mCRPC #RLT #Olaparib #radium223 @urotoday @oncodaily @OncoDailyGU @DrRanaMcKay

And confirmed again that #PSA Nadir (cut-off 0.2 l) is a strong prognostic factor and predictor of oncological outcomes OS, CRPC-FS in patients with #mAPMS PC. A higher proportion of patients treated with ADT+ARPI compared to ADT+Docetaxel or ADT+D+ARPI achieved a deep PSA response - would be very interested in seeing the breakdown per treatment in both low and high volume categories, but I guess the nadir achieved reflects predominantly more aggressive disease rather than a treatment effect #ProstateCancer #UrologicOncology #PSANadir #Biomarkers @oncodaily @OncoDailyGU @urotoday @chavarriagaj

While DASL-HiCaP reinforces ARPI intensification, important questions remain @Silke_Gillessen Why positive #STAMPEDE/#DASL-HiCaP, yet not #ENZARAD? Which high-risk patients benefit most? Primary vs salvage RT? And how does #PROTEUS reshape intensification around surgery? #Enzalutamide #Apalutamide #Darolutamide ❓Different risk definitions and primary vs salvage settings complicate comparisons. ❓Who truly needs ARPI intensification, and when? What is the additive role of #RadiationTherapy and #RadicalProstatectomy ❓With PROTEUS extending the discussion to prostatectomy, better patient selection and identifying the best prognostic and predictive biomarkers reflecting specific early/local/locally advanced indications remains key #ESMO26 #ProstateCancer #UrologicOncology @oncodaily @OncoDailyGU @urotoday

Honoured to have had the opportunity to present "Optimal induction treatment in non-metastatic bladder cancer" at #ESMO26 in Madrid! 🚨 Does a deeper pathological response translate into greater survival benefit in perioperative MIBC? An intriguing pattern across four phase III trials: 🔹 NIAGARA: ΔpCR +10 pp | EFS HR 0.68 🔹 KEYNOTE-B15: +23 pp | HR 0.53 🔹 VOLGA: +32 pp | HR 0.51 🔹 KEYNOTE-905: +49 pp | HR 0.40 Larger absolute pCR gains appear to track with greater EFS benefit. But the comparator matters! KEYNOTE-905 and VOLGA used surgery-based controls without neoadjuvant systemic therapy (control "pCR" ~8–9%), whereas NIAGARA and KEYNOTE-B15 used active neoadjuvant cisplatin-based chemotherapy. Thus, the magnitude of ΔpCR is not directly comparable across trials. The key question: Can pCR, combined with utDNA + ctDNA clearance, become a reliable tool for response-adapted treatment intensity? ⚠️ This is an exploratory, trial-level observation—not proof that pCR improvements cause EFS benefits or that pCR is a validated surrogate endpoint! Better biomarkers could help us maximize cure while minimizing overtreatment. 🔬 #ESMO26 #BladderCancer @weoncologists @urotoday @Uromigos @OncoAlert Faculty: @DrChoueiri @tompowles1 @apolo_andrea @neerajaiims @montypal @Silke_Gillessen @AOmlin @brian_rini @PGrivasMDPhD @TiansterZhang @amerseburger @DrDanielHeng @HHammersMD @scocmem @AndreaNecchi @AmandaNizamMD @drenriquegrande @nataliagandur @AlbigesL @cdanicas @declangmurphy @seanmmcbride @shilpaonc @DrRanaMcKay @scserendipity1 @DrYukselUrun @ravikanesvaran @BraunMDPhD @EfstathiouEleni @mzugman @crisbergerot @Markuseckstein3

Darolutamide with ADT and locoregional radiation therapy (RT) in high-risk prostate cancer (DASL) showed Darolutamide reduced the risk of relapse and time to CRPC. Tolerably was good. It’s imporant to identity which patients benefit, as mortality is low, and whether latest staging with PSMA-PET changes things #ESMO26 @Silke_Gillessen @ChrisSweens1

Great to finally see the results of the #PROTEUS substudy comparing #perioperative Apa+ADT with the true SOC (RP w/o neoadjuvant treatment) - which should at least reduce some concerns related to the active comparator with ADT in the main study that didn’t reflect what we actually do as a standard in HR patients #ESMO26 Outcomes in all #endpoints significant improved ✅EFS: HR 0.38, 95% CI 0.26–0.55 ✅MFS: HR 0.49, 95% CI 0.26–0.90 ✅TT 1st Sub Rx: HR 0.29, 95% CI 0.20–0.43 The question remains which of these outcomes are most clinically relevant and meaningful #PROTEUS #ProstateCancer #UrologicOncology #Apalutamide #RadicalProstatectomy @oncodaily @OncoDailyGU @urotoday @Albert0Briganti @JNJInnovMed

Couldn’t agree more @niklas_kluemper It is our responsibility as urologists to make sure that patients receive the most appropriate treatment and that we consider perioperative treatment with #EVP. Most of the patient patients eligible for #RadicalCystectomy will also be eligible for EVP #BladderCancer #UC #Urology @oncodaily @OncoDailyGU
KEYNOTE-905/EV-303: a major step forward for cisplatin-ineligible MIBC. Perioperative enfortumab vedotin + pembrolizumab (EVP) significantly improved EFS, OS and pCR versus radical cystectomy alone. At 2 years: 🎯 EFS 74.7% vs 39.4% — an absolute difference of 35.3 percentage points. ➡️ Roughly 1 in 3 more patients event-free with perioperative EVP!!! And 57.1% achieved pCR, compared with only 8.6% with surgery (complete TURBT..) alone. Importantly, benefit was broadly consistent across key subgroups in this older, comorbid population, including patients with ECOG PS 2. From my perspective as both a urologist and GU oncologist: if a cisplatin-ineligible patient is considered fit enough for radical cystectomy, the patient can (mostly) also receive at least 3 cycles of neoadjuvant EVP! We finally have the first approved perioperative regimen for cisplatin-ineligible MIBC — and an effective regimen that we should integrate into clinical practice! @Markuseckstein3 @ViktorGruenwald @JoshMeeks @UniklinikBonn @OncoAlert @urotoday @PTarantinoMD @Uroweb @drenriquegrande @PGrivasMDPhD @amerseburger @DrYukselUrun @tompowles1 @Uromigos @OncoBellmunt @montypal @DrChoueiri @neerajaiims @shilpaonc @DrRosenbergMSK @Dr_Aggen

Great work and important insights again from @Markuseckstein3 for future clinical development Really interesting work in a challenging #bladdercancer subtype #SmCCB #BladderSmallCellCarcinoma 👉The striking divergence between genomic NECTIN4 amplification and actual target expression is an important reminder: we cannot simply extrapolate biology from conventional UC to SmCCB 👉 DLL3 and SLFN11 may point us towards a more biologically rational treatment strategies #UrothelialCarcinoma #TargetedTherapy @oncodaily @urotoday 👇
Small cell carcinoma of the bladder: why ADC targets from conventional UC do not simply carry over, and what may work instead 🧵 New in @Histo_Journal: 88 SmCCB from Milan and Erlangen, 79 fully profiled by IHC for ASCL1, NEUROD1, POU2F3, YAP1, HNF4α, NE and tuft cell markers, plus 7 therapeutic targets and NECTIN4 FISH. 🔬 Two phenotypes 🟢 High-NE (57/79): ASCL1-driven, NEUROD1-driven or mixed, diffuse synaptophysin, chromogranin and CD56 🟠 Low-NE (22/79): POU2F3-driven, YAP1-driven or TF-negative, weak or absent NE markers, significantly associated with admixed urothelial, glandular or squamous histology 🎯 ADC and BiTE targets, one by one Nectin-4 (membranous) • Completely negative in 75% (60/80) • Median H-score 0, mean 11.6, max 220 • Higher in Low-NE than High-NE (median 1.5 vs 0, P<0.001), consistent with retained urothelial differentiation NECTIN4 amplification (FISH) • 16/73 (21.9%), a frequency comparable to conventional UC • No difference between High-NE and Low-NE (P=0.835) • No association with membranous protein (P=0.099) TROP2 • Completely negative across the cohort HER2, FOLR1, Claudin 18.2 • Rare and weak expression only DLL3 • Positive in 35/43 evaluable High-NE cases (81%) • Nearly absent in Low-NE (P<0.001) • Relevant for DLL3-directed BiTEs such as tarlatamab and for DLL3 ADCs in development SLFN11 • Median H-score 125, significantly higher in High-NE (P=0.006) • Supports testing DNA-damaging strategies in this group 💡 Key takeaways 1️⃣ High-NE SmCCB is largely Nectin-4 protein negative, yet carries NECTIN4 amplifications at rates similar to conventional UC. The amplification is present, the protein is not. 2️⃣ Our hypothesis: this points to NE transdifferentiation from a urothelial precursor that already harboured the amplification. The gene status is retained, while the urothelial protein program, including Nectin-4, is switched off during lineage change. The equal amplification rate in both phenotypes fits this model. It needs confirmation in paired and clonal analyses. 3️⃣ Practical consequence: NECTIN4 FISH alone should not be read as a surrogate for target presence in SmCCB. Gene status and membranous protein both need to be assessed. 4️⃣ Enfortumab vedotin benefit cannot be assumed in High-NE SmCCB. Low-NE tumours with residual urothelial features remain the more plausible candidates for UC-type ADCs. 5️⃣ High-NE SmCCB behaves biologically like SCLC: DLL3-high, SLFN11-high. This makes it a rational population for SCLC-directed strategies. 6️⃣ A tuft cell-like subset exists: POU2F3-driven tumours co-express POU2AF2 (11/16) and ChAT (13/16), with preclinical mSWI/SNF vulnerabilities. 7️⃣ A simple workflow (NE markers ± TF panel) can stratify SmCCB in routine diagnostics today. ⚠️ Limits: retrospective, TMA-based, no treatment or outcome annotation. Treatment-annotated and paired pre/post-therapy cohorts are the next step. Big shout out to Nazario Tenace from San Raffaele he drove this work as fellow. Bright future for this talented pathologist. Shout out also at great collaborators from Sann Raffaele @AndreaNecchi @Albert0Briganti @F_Montorsi and @mauriziocollechia @maurilioponzoni @claudiodoglioni and @UroMoschini Great collaboration between San Raffaele Milan and UK Erlangen, led by Nazario Tenace, with @andreanecchi 🔗 doi.org/10.1111/his.70284
