8 weeks for #HCV?
In RESOLVE, 8 weeks of sof/vel was non-inferior to 12 weeks in treatment-naive adults without cirrhosis (SVR12: 98.8% vs 99.0%). @ Amit Goel @aiims_newdelhi @SGPGI @kgmu_medical @INASL_Liver @TheLancet

🧬 Our new paper in @JCEH_Hepatology is out!
In 69 North Indian MASLD patients genotyped for 12 candidate SNPs, we found that APOC3 and PNPLA3 operate on distinct genetic axes:
🔸 APOC3 rs2854116 — enriched in MASLD vs Indian population (OR 1.93, P<0.001) but NO fibrosis association → susceptibility variant
🔹 PNPLA3 rs738409 — strongest fibrosis predictor (OR 2.89, P=0.006) → severity variant
🔥 Carriers of risk alleles at ALL 3 loci (APOC3+PNPLA3+SAMM50) had 90% fibrosis prevalence (OR 13.7, P=0.0001)
Who gets the disease ≠ who gets worse. Ancestry-specific genetic data matter.

HCC in GT 3 HCV 🇮🇳
kwnsfk27.r.eu-west-1.awstrac…
De novo HCC post SVR-12 in HCV compensated cirrhosis. was in 128/3994 (3.2%), median follow-up of 4 years, an incidence of 985 per 100,000 person-years; 77.3% men, 71.6% genotype-3, BCLC stages were A (60.2%), B (28.9%), and C (10.9%).
Liver stiffness at SVR-48 (aHR 1.01, 95% CI: 1.001–1.030, P = 0.021) and ALBI score at
SVR-12 (aHR 1.31, 95% CI: 1.1–1.4, P = 0.034) predicted de novo HCC. Recurrence rate ⬇️patientswith de novo HCC than in those with HCC

🚨 Recompensation in decompensated AIH cirrhosis
🔹34% achieved Baveno VII recompensation
🔹 Transplant-free survival was 97% vs 63%* with vs without recompensation
🔹 20% re-decompensated
Recompensation in AIH is real but dynamic.

🇮🇳 Real-world data from >1000 patients with MASLD/MetALD
MRI-PDFF phenotyping + 🧲 MRE anchor
🔍 Key takeaways:
✔️ Similar CMRF & AF burden
✔️ FIB-4 & NFS perform well
❌ APRI underperforms
💡 Simple NITs even for MetALD
