Plan is heavy on quick wins (mostly paint), and I'd like

No other industry where fatalities are possible operates in this way. I spent most of my career in heavy industry. Fatalities can occur even when equipment is used as intended.
But in literally any other industry, if a fatality did occur it would cause all operations to cease immediately until the root cause was found. If an operator was found to have been reckless, she would have been fired.
If the operator was not at fault and the the cause was equipment or procedure, the equipment or procedure would be changed.
In no scenario would it be just “yip, that happens sometimes. Carry on”. No regulator, no labor union, not even a customer would accept that answer for a second. A leader who tried to pull that would be fired.
Only on policing do we accept death as a normal by produce.
