OP with Alzheimer’s dementia & recurrent falls referred. My suggested intervention below as she could not remember she needed her walker. Follow up recently no further falls. I love my job❤️😊👍🏾@GeriSoc @LivHospitals @Mersey_Care @NHSCandM
