When should we pull the trigger for VV ECMO in ARDS patients? A consideration for shops that do not offer ECMO is to get on the phone earlier to either allow for safer transport or to allow the team to arrive to cannulate. 🎩 tip to the authors!

#ECMO & tracheal procedures? ELSO Registry analysis
🪦 64.3% survival to discharge
🫁 among surgeries, tracheal resection most common, higher survival vs airway reconstruction/injury repair
🫁 for bronchoscopic procedures, tracheal stent had favourable survival, tumor debulking poor
🩸hemorrhagic complications common in prolonged runs, associated with worse survival

#echofirst PV is not that friendly to the TEE imager - often the only way to get it is using a non-standard TEE view (especially for Doppler measurements)
One of them (often the only one) is the deep TG PV LAX 👀👉
From the deep TG LAX at 0, just rotate the probe to 30-90.

ICU Circulation Secrets:
The phenomenon of "hemodynamic incoherence" is observed when microcirculatory dysfunction persists despite the restoration of macro-circulatory indices
In this case, MAP was 88 mmHg & cardiac index was 2.8 l/min/m2 on a small dose of norepinephrine
