Texas was built by hope. By people who had no business believing — but believed anyway.
To every Texan who’s ever wondered if anyone in power is paying attention: We are taking our state back.
Because a little bit of hope is a dangerous thing.

At 28 years old, a young man decided to walk 25 miles in a single day. Not for exercise. He was walking the length of his district, stopping along the way to talk to people and hear what was happening in their lives.
By the time he was halfway through, something was very wrong. He was exhausted, nauseated and vomiting. But he kept walking.
He finished the 25 miles and went home. Then he slept for 36 hours. His parents, alarmed by this, took him to the emergency room where they found out his blood sugar was 900.
The young man had undiagnosed Type 1 diabetes and was in diabetic ketoacidosis, a life-threatening condition that can lead to coma and death without treatment. He was then rushed to the ICU.
But thankfully, that young man survived, and story doesn’t end there.
Once he was able to leave the hospital, he needed regular insulin to stay alive. His first 30-day supply cost $684. Young men his age don’t carry that kind of money. So he put it on a credit card.
And that experience changed the direction of James Talarico’s young life.
Since then, James has been fighting for people who need insulin and other lifesaving medications in order to live, and pushing for legislation to make those drugs more affordable.
Working for the people, not Big Pharma. James Talarico is who Texas needs.

please stay clear of AI to write letters to medical journals, write papers, essays, etc.
would much rather have flawed humans with incompletely/maybe not well expressed ideas, thinking/challenging/debating what we know, than algorithmic fakery. we will make ourselves stupid

Talarico: "Billionaires are pouring an unprecedented amount of money into Texas to try to buy this Senate seat ... they are hoping that Texans will care more about these manufactured culture wars than actual wars. They're hoping Texans will care more about pronouns than prices. But I think Texans see through this distraction."

These letters are the tip of the iceberg of the prob w CHAMPION (and OPTION, btw)
CHAMPION will be used in EBM classes as an eg of biased design and interpretation
A colossal embarrassment for the academics involved
Note what BSX stock did the Monday after the *positive* trial was presented

Published in the NEJM, three letters raise important points about CHAMPION-AF:
1) The primary endpoint includes cardiovascular death (42% of events), which is a “dilutive” component that does not characterize either treatment arm and favors a finding of noninferiority.
2) Focusing on ischemic stroke reveals a hazard ratio of 1.61 (95% CI, 1.00–2.59), with no difference in hemorrhagic stroke. This is somewhat the opposite of what one would expect from percutaneous LAAC.
3) The number of events was so much lower than anticipated that, while maintaining the same noninferiority margin, the trial effectively allowed a doubling of events while still declaring noninferiority.
4) It is inappropriate to consider the risk of intracranial hemorrhage solely as a function of oral anticoagulant exposure, while ignoring predisposing structural conditions such as cerebral amyloid angiopathy.
