Something an EP attending taught me in fellowship that has stuck with me: If someone has permanent Afib, Afib with rapid rate is their "sinus tachycardia". Anything that would cause sinus tachycardia in someone without Afib (infection, stress, dehydration, etc), would cause "AF RVR" in a person with Afib. It is not a new de novo condition for them. Stop chasing the AF RVR, treat underlying condition, the RVR resolves on its own. #cardiology #EP #Afib

Answer Key: Exclamation Point Sign and Mickey Mouse Sign. Exclamation Point Sign is when the Gallbladder neck points towards the Portal Vein. Mickey Mouse Sign is the combination of the CBD, Hepatic Artery, and Portal Vein that looks similar to the distinctive shape.

Always expect the unexpected in EM. 20s ♂️-"I have GC again." "Hurts so bad when I pee." Easy one, right? Get a urine, tx for STD and out the door? See thread. #FOAMed #Medtwitter @AshleyGWinter @jminardi21 @medpedshosp @grepmeded @thepocusatlas @ACEP_EUS @ACEPNow

Provocative take: Requiring "clinical signs" to diagnose tamponade is like waiting for a free wall rupture to diagnose an MI.

It’s got to the point with certain new resus patients or ICU admissions where I bring the ultrasound with me when I examine the patient for the first time. Not doing it feels like a massive waste of time and I’ll just be playing catch up later on

Here's some good news this week.. 1. Ted Lasso returns on 3/15 and we could all use a good laugh. Football is life. 2. The results of the match are not that "the sky is falling". We were growing too fast as a specialty and this can be viewed as a market correction. @ACEPNow 1/
