If you have a billion dollars, you cannot buy good medicine. Because good doctoring has three components — and none of them are visible from the outside. Technical skill: Can your surgeon actually operate? Other surgeons barely know. You don't. Relational skill: Does your doctor listen? Patients are actually the best judges of this one. Cognitive skill: Can they appraise evidence and apply it correctly to you — not to the average trial patient? No market signal identifies all three. — Dr. Anil Makam, UCSF

Why the NHS is failing #826363839 1. Fragmentation of care. It’s almost impossible to read a clinical story in GP notes now. It’s messages and anima requests and “failed encounters” when someone can’t answer the phone. (1/n)

I’d like to congratulate @RyderCupEurope on their victory. Your team play the first few days was sensational. More importantly, I’d like to apologize for the rude and mean-spirited behavior from our American crowd at Bethpage. As a former player, Captain and as an American, I am ashamed of what happened. #RyderCup

Enough of this. FFS. 🙇🏼‍♂️🙇🏼‍♂️🙇🏼‍♂️🙇🏼‍♂️🙇🏼‍♂️ ACPs are great but ARE NOT DOCTORS, and most of them know they aren’t and are entirely happy not to be.
I can do most things an astronaut can. Except go to space.

I was the on call Anaesthetic resident for emergency theatres last weekend and a trainee SCP was first assist for certain cases e.g lap appendix and was doing other cases e.g abscesses with local supervision. I felt really uncomfortable with this. 1/n

Did a Ray Cooney farce with this comedy genius. He was playing the butler. Stole the show every single night without even trying. One of the kindest, gentlest, most generous actors I had the good fortune of working with. A true comedy legend. RIP Brian Murphy.

Hear hear. Backlash on fewer targets is misguided. Bolder cutting of targets required; doing half of the wrong thing is better, but still not great. Targets often / usually distort clinical priorities. Performance measures should focus on failure demand.
We have to do things differently. I'm instructing the NHS to fix the fundamentals. It needs to deliver on patients’ priorities – shorter waiting times, being able to get a GP appointment and ambulances that arrive on time. telegraph.co.uk/news/2025/01…

The original vignette was shocking in its failure to recognise the strength of primary care and general practitioners - strong primary care = better health system, better care, lower costs. I haven’t seen anyone say “Sorry”. Vignette just withdrawn. 🤯🤷‍♂️

@NHSEngland should respond to this. I saw the original vignette and nearly cried. If they think that’s the future of medicine, count me out - as a doctor and as a patient. Thank you @JohnLauner 👏👏@NHSEngland you could employ people like John!

In his wonderful, wide-ranging new book Rebalancing Medicine @stokemacca describes how NICE was born in the late 1990s out of a cost-benefit debate about interferon for MS. There is no better insider account of what followed and how we got to where we are.

Head of the OBR, Richard Hughes, say the treasury broke the law by not disclosing a multi billion pound black hole in public finances. The casual suggestion that Labour lied about the conservative overspend and obfuscation by Tory MPs (and Tory media) is completely unfounded.
