Donor BMI rises, liver biopsy use in LDLT is plummeting.
🇺🇸 In 7,942 LDLTs (2000–2024), donor BMI rose 24.6→27.6, while biopsy use fell ~50%→6%. Higher BMI was not linked to worse recipient outcomes.
Time to standardize steatosis assessment?

Can a deceased donor–derived predictor guide decisions in living donor LT for ACLF? 🧐
In 178 LDLT recipients, SALT-M showed limited discrimination (AUROC 0.56).
Model failure or simply a different patient population? Use with caution ☝️

Large multicenter study from India and Singapore.
Highlights:
TIPS is excellent for AVB (>90% control); decent for Ascites (50% control);
For HRS-AKI: survival 52% compared to 73% in no HRS.
First study on ACLF type A (APASL)- TFS 33%
FIPS and MELD are valid in Asian cohorts

TIPS in higher-risk patients?
Multicentric data of 207 patients, 96% remained free of variceal rebleeding, while 51% achieved ascites control. The HRS-AKI and ACLF findings raise important questions

Why does portal vein thrombosis matter in hospitalized patients with cirrhosis?
In the global CLEARED cohort investigating 7,721 patients across 37 countries, PVT affected 1 in 10 and was linked to more AKI, severe HE, and higher 30-day mortality.

Together we (@serperm) prevented falls in patients with cirrhosis.

1/ It's #MalnutritionAwarenessWeek. Malnutrition is common in cirrhosis, and we've known it for decades. What we haven't done is build it into routine care the way we did carvedilol and pHTN. We need to routinely screen for it and proactively manage it when present.

6.9% of LT candidates were delisted after clinical improvement. But when is “better” good enough to leave the waitlist?
Khan & Aby: recovery varies by etiology and timing, and MELD improvement may not equal durable recompensation.
Better markers needed.

#Thrombophilia ≠ automatic exclusion from living liver donation.
Isolated heterozygous Factor V Leiden or Prothrombin G20210A may be acceptable with standard thromboprophylaxis. Prior VTE or high-risk genotypes remain red flags.
