What is the role of dynamic substrate changes in facilitating conduction delay & reentry in VT circuits?

unsurprising clear benefit for ablation in ⬇️AF burden and ⬆️ in quality of life vs SHAM. follow up short bw M3&M6 but given overwhelming science & clinical evidence behind PVI difficult to prolong F/Up much longer #ESCCongress @DrJasonAndrade jamanetwork.com/journals/jam…

The first sham-controlled AF ablation trial has been done. Twenty years (and millions of procedures) after we started doing this procedure.
The study has limitations but it is a big step forward. I need to take some time to sort it out.
For now, though, our profession should have been bolder and humbler about this much sooner. #ESCCongress
It was possible
It was ethical
It settled the issue
It was nothing for EPs to be scared of
It wasn't "EP's Orbita" 😜

I believe PVI is an excellent treatment for symptoms, but how, in the face of MIST & ORBITA, can we still believe that *our* procedure is so important/ fantastic that a sham trial is unethical??
