The dashed curve shows patients with a hypertensive response to exercise (HRE), defined as the highest tertile of the systolic blood pressure rise relative to oxygen uptake (SBP/VO₂ slope). The solid curve shows the other two tertiles. Event-free survival was lower in the HRE group (log-rank χ² = 8.31, P = 0.004). Numbers at risk are shown below the plot at 0, 12, 24, 36, and 48 months. In a separate Cox analysis, a steeper SBP/VO₂ slope was associated with a higher hazard of the composite outcome (adjusted HR 1.13 per SD, 95% CI 1.05–1.20), accounting for age, sex, BMI, type 2 diabetes, established heart failure, peak SBP, and workload.

HFpEF-ABA Score and Finerenone in Patients With Heart Failure and Mildly Reduced or Preserved Ejection Fraction: Insights From the FINEARTS-HF Trial
Despite a proven diagnosis of HF with mildly reduced ejection fraction/HFpEF, one-third of participants in FINEARTS-HF had an HFpEF-ABA score <75%, yet the therapeutic effect of finerenone was consistent across the range of HFpEF-ABA scores included. 🎙️ #JACCばらん Ep.86 | PFAはマーシャル静脈まで届く？
🎯 「肺静脈隔離（PVI）のためのpulsed field ablation（PFA）は、意図せずVein of Marshall（VoM）にも影響を及ぼしているかもしれない」という論文です。
今回のゲストは 中谷 洋介 先生（群馬大学 @yosuke3gbst）🇯🇵
PFAによるPVIを受けた72例を対象とした前向き観察研究。FARAPULSE、VARIPULSE、PulseSelectの3つのPFA systemを用い、VoM内にmicroelectrode catheterを留置して、PFA中のVoM電位をリアルタイムに評価しました。
注目は、PFA直後には44%でVoM電位が変化したものの、手技終了時まで変化が残ったのは14%だったこと。そしてVoMへの影響を左右したのは出力エネルギーの強さ以上にPFA electrodeとVoMとの「距離」でした。

🚨 Key insights from our invasive hemodynamics #HFpEF study (n=1,069):
🌊 ~21% of cases have a prominent V wave (≥10 mmHg > mean PAWP), indicating operant left atrial stiffness
📉 Correcting PAWP for the V wave normalizes PVR and halves CpcPH prevalence (60% ➡️ 29%).
💔 Despite normal corrected PVR, these patients have worse RV dysfunction and 2x the risk of HF hospitalization/death, which may happen independently of pulmonary vasculature.
👉 Clinical Takeaways for RHC:
✅ Don't overestimate: Factor the V wave into TPG/PVR to avoid overestimation of pulmonary vascular disease in HFpEF.
✅ Risk stratify: Prominent V waves = high-risk patients.
✅ Future targets: We need therapies targeting LA compliance as well, not just the pulmonary vasculature.

HFpEFで「CpcPHだから肺血管治療?」と考える前に。PAWPのV波、見ていますか?🫀
Mayo Clinicの侵襲的運動負荷試験(HFpEF 1,069例)より。
＜ポイント＞
①HFpEFの約21%で、PAWPのV波が平均PAWPより10mmHg以上高い。左房の硬さ(LA stiffening)を反映し、約75%はablation歴のない非医原性。
②V波補正PAWP(平均PAWP+V波増高分の1/3)で計算し直すと、V波群のCpcPHは60.2%→29.3%と半減。補正PAWPはAUCともよく一致 (R²=0.96 vs 0.86)。
③補正後PVRはV波なし群と同等(1.38 vs 1.25 WU)。それでもPAコンプライアンスは低く(3.03 vs 4.90 mL/mmHg)、TRや右室機能障害が多く、HF入院/死亡リスクも高い(調整HR 2.09、平均PAWP調整後も1.61)。
👉 肺血管に病変がなくても、左房の硬さだけで右心不全は起こりうる
＜臨床にどう活かすか＞
✅ RHCでは、TPG・PVRの算出にV波を組み込むべき（肺血管疾患の過大評価を防ぎ、HFpEFで肺血管治療薬が登場しつつある今、治療の誤選択を避けるためにも）
✅ V波が目立つ患者はハイリスク群
✅ 治療ターゲットは肺血管ではなく左房コンプライアンスかもしれず、今後の研究課題

【今日の循環器論文 心不全】
HFpEFの診断で「運動負荷心エコーは侵襲的検査の代わりになるか」を検証した多施設研究（482例、うちHFpEF 386例）。
・従来スコア（H2FPEF/HFA-PEFF/HFpEF-ABA）＋推奨どおりの運動負荷心エコーでは感度55-60%、正確度61-67%
・安静時の左房コンプライアンス（LA reservoir strain÷E/e'）を加えると感度84-85%だが、偽陽性が31-43%に増加
・確定用と除外用で別カットオフを使う運用（運動時E/e'≥13.8 またはLAコンプライアンス≤1.6%で確定、両方が陰性なら除外）にすると、確定群の感度95-99%、侵襲検査が必要な例は約60%→約30%へ
💬運動負荷エコーが陰性だったから大丈夫、と言いたくなる場面は多いです。一方でこの結果を見ると、グレーゾーンを正直に「判定不能」と扱う運用のほうが安全そうです。

PFAのMarshall静脈（VoM）への影響をまとめました。
VoMを修飾できれば治療成績は良さそうですが、狙って修飾するのは難しく、VoMへのエタノール注入はまだ重要な治療オプションですね。
