FDA approves teclistamab plus daratumumab (Tec-Dara) combination for myeloma for patients who have had at least one prior line of therapy.
Note: Always use Tec-Dara with monthly IVIG. It’s not optional.
That was fast! And this is unprecedented curve is why.

Early results
✅ 60% of patients showed improvement/resolution of dysgeusia after 7 months
✅ WETT detected changes earlier than standard CTCAE grading
✅ More objective than patient-reported outcomes

Most patients, the default is a quadruplet.
Either Dara or Isa can be used as anti CD38 depending on access and availability
Prefer DaraVRd or IsaVRd rather than KRd based regimens
For standard risk early or delayed transplant depending on patient preference, response to induction, and tolerability of induction.
For standard risk doublet maintenance if MRD positive and/or Anti CD38 naive
We are awaiting results of trials to limit duration of maintenance, for both Len and Anti CD 38. Stay tuned.
