The TAVI-PCI dilemma: is there a right sequence? How would you treat this patient?
🔗pcronline.com/Cases-resource…
A 79-year-old man with severe symptomatic aortic stenosis and concomitant coronary artery disease presents a common but challenging dilemma: should the #coronary lesions be treated, and if so, should #PCI be performed before, during or after #TAVI?
View part 1⃣ of this case submitted by Philipp Seppelt and join us on 18 November for a #PCRwebinar on TAVI in patients with concomitant coronary artery disease where the team will be discussing part 2⃣!

In this prespecified FAVOR III China analysis of 3,455 patients, lower post-PCI μQFR was associated with a higher 5-year risk of MACE (aHR 1.30 for lowest vs highest tertile), driven mainly by ischaemia-driven revascularisation.
Discrimination was modest, supporting use as a continuous metric rather than a binary target.

Rapid CYP2C19 genotyping can inform P2Y12 inhibitor selection after PCI.
In this study, delayed de-escalation to clopidogrel in loss-of-function carriers was not associated with a significant increase in ischaemic or bleeding events.
