This patient is ill! Changing to Humira expecting a different outcome is ludicrous and dangerous. If IFX doesn’t work- Stelara is indicated !!
21 yo F w longstanding SB/colonic Crohn's, failing IFX despite trough 15.4, recent 5 lb weight loss (BMI 14.8), hospitalized with abd pain. @blueshieldCA denies Stelara due to policy requiring failing Humira first. Won't even honor peer-to-peer request. @IBDMD #fightwithme

1 of 2: Pt w/active #crohns doing well on adalimumab + 6MP became profoundly tired and nauseated @2 mos. Assuming these to be 2° to worsened disease, MD increased dose. We saw pt at 6 mos, D/C'd 6MP and pt returned to normal. Labs confirmed Pt is mismetabolizer. @CedarsSinai #IBD

2 of 2: While 6MP adverse effects typically begin immediately, in this case they were delayed. Before assuming worse disease and escalating Rx, try to discontinue 6MP. @CedarsSinai #IBD @cedarssinaiibd

1of2: UC pt in clin remission at 40mgQOW adalim (bld lev=5) but flared. Responded again at 40mgQW (blood lev=23) but flared. MD plan=change Rx class but w/o checking blood levs. In fact level=9 so Rx dose can be increased more to get to previous response level. @CedarsSinai #IBD

2of2: Yes higher doses can be associated w/inc side effects, but be sure to get levels to give Rx a fair chance before eliminating 1 of the Rx options. Also remembe, can stay in a the same class with a Rx that gets to higher levels, eg, infliximab or biosimilar. @CedarsSinai #IBD
Good job @CedarsSinaiIBD team.

Group photo of @AmCollegeGastro Board of Trustees meeting here at #ACG2018. It’s an honor to be included among these distinguished leaders in our field to whom I look up with admiration. Today we’re discussing ways to further enhance the science and practice of Gastroenterology.

ASAs can have paradoxical effects, ie colitis exacerbation, pancreatitis, pleuritis etc, w effective tx by d/c of ASA. In lung, however, cryptogenic organizing pneumonia seldom resolves by d/c of ASA or even pulse steroids. Prolonged steroid treatment is almost always required.
Single cell RNAseq has helped @Suzanne_Devkota come up with very exciting findings regarding creeping fat in relation to the #microbiome. @CedarsSinaiIBD @CedarsSinai

Pt w/clinical & lab signs (CRP>150) of severe CD to the point surgery considered & hi concern re infection. Interestingly, little inflammation seen on endo. MRE showed major submucosal inflamm w/marked edema. Tx=anti-TNF. Such disconnects are uncommon but do exist. Stay vigilant.
