An 85-year-old woman with severe TR, symptomatic heart failure and reduced right ventricular function presents a challenging transcatheter treatment decision. Although the tricuspid anatomy appears favourable, several clinical factors add complexity to the choice of strategy...
This case illustrates that optimal device selection cannot be reduced to anatomical feasibility, individual risk scores, or the pursuit of maximal TR elimination alone, and underscores the central role of an experienced multidisciplinary Heart Team.

In the SPIRARE II trial, mechanical aspiration thrombectomy with the Vertex Pulmonary Embolectomy System met its prespecified safety and effectiveness endpoints in intermediate-risk pulmonary embolism, with improved right ventricular function and a low rate of major adverse events.
