Why can't you use direct oral anticoagulants (DOACs) in patients with mechanical valves (MVs)?
DOACs have been one of the most important advances in my career. And yet, the presence of a MV is one of the few contraindications.
The reason highlights the unique nature of thrombus formation in those with a MV and provides insights into the evolution of human hemostasis.

Challenging Cases:
84yo WM w/AS, did TAVR. Used single Proglide in LCFA. Proglide got stuck and foot plate mechanism would not disengage from Ca+. We deconstructed and safely removed from the arteriotomy. Fantastic tips & tricks, big credit to Dr Amy Daliman @TAVRBot

Echo is one of the more skilled ultrasound exams you can do. Having learnt it and now teaching it, I see the same mistakes being made over and over again. I have put together thread on beginner mistakes in echo. #tweetorial #POCUS #FUSIC #askpocus

Absolutely great video to illustrate the aortic and mitral valve anatomy, leaflets and continuity. Imagine you are looking from the ventricular side. Note the relation between the AMVL and the NCC (3-7 O’Clock) and the LVC (11-3)

💡 30y old male referred to our centre for severe mitral regurgitation.
Classic Barlow valve with mixomatous degeneration as shown by 2D/3D TOE:
✔ Huge annulus dilatation (48 mm - intercommissural)
✔ Main lesion: wide P2 prolapse, small P3 prolapse
✔ Secondary lesion: A1 prolapse
✔ Mitro-aortic angle: 110°
✔ SAM-risk: ++++
Surgical approach in right mini thoracotomy:
✔ P2 resection
✔ A1 neochordae implantation
✔ Complete annuloplasty (38 mm ring)
✅ No residual MR
✅ No SAM
✅ 1 cm coaptation lenght
Stunning resolution with Vivid E95 #GEcasesTOE and excellent result!

I use ultrasound for all my venous and arterial access (IJ & femoral) , including L subclavian access for pacemaker implantation. Lower risk of pneumothorax. Any #EPeeps routinely use u/s for subclavian access? This paper explains this very nicely 👇

" SUPPORT "
Everything in life is only possible if you have the right support.
In Interventional Cardiology it is no different.
Unfortunately, the right material for optimal support during PCI may not be available or may be too expensive.
I would like to share our recent article published in Current Problems in Cardiology: "Catheter Hole Support" a novel technique that provides very high support and stability when needed.
I have no words to thank the brilliant Professor Emmanouil (Manos) Brilakis for all the necessary support for this publication.

Support for my argument that even if LAAO reduced stroke (it doesn't against warfarin), it could not reduce all-cause death.
Green line is stroke --> small cause of death in patients with AF
