The FDA has the chance to put its new Rare Disease Framework into action by granting the requests in the 313-pg Citizens' Petition and 330+ pg legal memo filed on Docket # FDA-2025-P-2109, much of which is based on real world evidence.
Like many #raredisease patients, people in the #ALS community had to become citizen scientists. We collected ~ 8 yrs RWE after a 28-week P3 RCT ended in 2020. Some data came from additional dosing in EAP. Some was contained in Electronic Medical Records documented by treating neurologists for years after the P3 trial ended in 2020. Some improvements in function were documented in videos.
That RWE showed unprecedented trach-free survival benefits as of October 2026. In the 10-person EAP:
🔹5 yr survival = 100% EAP vs <20% ALS natural history
🔹5 yr median survival benefit = 7.5 yrs EAP vs 2.5 yrs ALS natural history
🔹5/10 people still alive, trach-free
🔹EAP's 51 patient treatment years ≈ patient treatment years in short 28-wk P3 trial
🔹Median not yet reached by those in the treatment arm in the trial and EAP (N-N) where 4/6 are still alive (range 7.5 - 9.5 yrs) with 81% lower risk of trach or death
Although OS and trach-free survival were pre-specified, the data were not yet ripe when the 28-week P3 trial ended. Now it is. The FDA has never considered this #RWE. The data confirm the AdComm testimony of renowned PI experts, treating neurologists and the post hoc findings of the 28-week trial.
The FDA approves cancer therapies based on 6 months of additional survival. NurOwn has extended survival by FIVE YEARS in a 100% terminal rare disease with no disease modifying treatments.
@FDA_KyleD @FDADrugs
The Rare Disease Framework will take years to implement -- years people with ALS don't have. Thus, we are asking the FDA and CBER to use its regulatory flexibility, act urgently, and grant accelerated approval of #NurOwn.

@laurencurehd, I am incredibly proud of you. It takes extraordinary courage to stand before the FDA and make the most painful parts of your life public. Behind your words are the sacrifices, the weight of Huntington’s disease, and the people you love whose futures hang in the balance. You carried all of that with strength, grace, and conviction. Thank you for giving our community a voice when the stakes could not be higher.

Today we remember the 11 worshippers at a synagogue in Pittsburgh who were murdered by a gun-wielding neo-Nazi. Eight years later, we keep the memories of the victims alive and express solidarity with the synagogue’s three congregations: New Light, Dor Hadash and Tree of Life. May the memories of those who perished in this savage antisemitic attack stay strong as we work toward a more just and peaceful world for all.

"Catch up" #CBER? For 500+ days, @FDADrugs has possessed unprecedented survival data in #NurOwn Citizens' Petition but has failed to respond.
Real-world Evidence from EAP:
• 100% 5 yr trach-free survival vs <20% in natural history
• >7.5 yr median trach-free survival vs 2.5 yrs natural history.
It's been 500+ days on @FDA's 180-day regulatory deadline to respond to the citizen's petition -- at the cost of 8k+ lives.

🧵 Some tidbits from the @FDA leadership roundtable at Day 2 of NORD’s Rare Disease Breakthrough Summit ... Center leaders included CDRH's Michelle Tarver, CBER's Karim Mikhail, and CDER's Michael Davis (moderated by Amy Comstock Rick of rare disease innovation hub):
If #CBER wants to honor the new framework, it can start by paying attention to the unprecedented survival data in the #NurOwn citizens' petition -- the ultimate clinically meaningful endpoint -- and the 2002 Public Comments at the AdCom explaining #ALS patients' tolerance for risk.
It's been over 500 days on a 180-day regulatory deadine. In that time, more than 8,000 Americans with ALS have #DiedWaiting.

During our 2026 #NORDSummit FDA Rare Disease Regulatory Roundtable, moderator Amy Comstock Rick asked the Directors of #FDA CDER, CBER, and CDRH: Where can patient voices make the most difference?
Center for Devices and Radiological Health (#CDRH): Identifying unmet needs, prototyping #MedicalDevices, involving patients in design of clinical trials to maximize participation and efficacy, gauging product quality once released
Center for Biologics Evaluation and Research (#CBER): Risk-benefit analysis of #GeneTherapy, defining acceptable outcomes and meaningful clinical endpoints
Center for Drug Evaluation and Research (#CDER): Patient-focused drug development meetings help explain which symptoms needs to be treated and which do not

Live from the 2026 #NORDSummit, @OtsukaUS CEO Tarek Rabah explained why #biopharma companies must prioritize rare disease research and treatment. Breakthroughs in #RareDiseases are transformative for science and medicine, and they are urgently needed.

ALT Quote from Tarek Rabah, President and CEO of Otsuka North America Pharmaceutical, at the NORD Breakthrough Summit: "The biggest breakthroughs happen where patients have been waiting the longest, where the science is the hardest, and where the risks are probably the highest. But that's also where the biggest reward is."

At the #NORDSummit, @CNBC's @BeckyQuick moderated a dialogue with the CEOs of NORD, @BioMarin, @JazzPharma, @OtsukaUS in which she challenged each of them to state their "wish list" for removing barriers to care for rare disease patients. Here were some of their answers:
• Cohesive patient data sets powered by foundations and advocacy groups
• More funding for organizations like #NORD to sustain that infrastructure
• Expanded newborn screening for rare, genetic conditions
• Improved medical awareness and education
• Diagnostic codes for more rare diseases
• More regulatory flexibility and fit-for-purpose standards for highly heterogenous diseases
• Consistency and predictability across #FDA review teams
• Exemptions from tariffs for orphan drug manufacturing
The overarching theme was that, to scale up #RareDisease research and treatment development, we need laws and regulatory policies that address the challenges patient communities and #biotech companies are facing, a #healthcare system with improved diagnostic capabilities, and well-funded patient organizations who can lead research efforts in their communities, in partnership with industry companies.
